There were several scoring systems to assess f***l incontinence but there were gross lacunae in them. For a long time, a need was felt for a new scoring system.

We developed a new statistically robust scoring system this year.

Yesterday, a new patient of recurrent a**l fistula (operated 6 times before) from Bangalore came to consult me. On meeting me,

Patient said "Dr, I was talking to some old operated patients of yours in waiting room. It seems that *you don't have patients, you have fans!*"

I took it as a flattery and ignored it. I instantly thought that ours is a doctor-patient relation and not cricketers-fan type relation.

Later in the day, a patient from Burdwan (W.Bengal) posted a review about us on Google. It was not much different from the previous reviews we usually get, but it raised a question in my mind.

*What is the difference between a client and a fan?*

On contemplating, it figured out that in all fields, *fans initially start as clients* ( even for cricketers, movie starts etc, people are initially clients and buy tickets for entertainment but become fans later).

*So, what turns clients into fans?*

I realised that when we do our work with passion, it turns into an art! And it is Art which creates *awe ( wow factor)* and turn clients into fans.

So, the conclusion was that when you do your work with passion, you have the capability to turn your clients into fans irrespective of your profession.

This phenomenon is universal and transcends all professions.

I got further convinced that

*Anything done with passion becomes an art*

We had organized an *INternational Summit for Treatment of A**l Fistula (INSTA)* on 17 July, 2021.

In this virtual conference, *27 Topmost surgeons from 19 countries gave lectures*.

INSTA-GFRI orations were delivered by

*Prof Steve Wexner* (Chief, Surgical disciplines, Cleveland Clinics, USA)

*Prof Ronan O'Connel* (Editor, Bailey & Love, Ireland)

*Prof Mario Pesactori* (Founding Editor, Techniques in Coloproctology)

*1406 surgeons from 43 countries* attended the conference live. There was Live online translation going on in Russian, Ukrainian and Spanish languages for surgeons in 7 countries.

This was the *largest conference on Fistulas till date*.

Am pleased to update that we have discovered a 'NEW ANATOMICAL SPACE' through which the A**L FISTULA spreads routinely but was not known or described before.

This discovery has been published in high impact US Radiology journal, 'Abdominal Imaging' ( Impact factor 2.43).

In 2015, we invented an highly effective procedure (TROPIS) for high complex a**l fistulas for which there was no good treatment available. We published it in 2017 (International Journal of Surgery, UK, Impact factor-3.35)

Now, we have published the Long term results study in in COLORECTAL DISEASE Journal, ( Impact factor 2.8), official journal of British and European Society of Coloproctology & 2nd highest ranking Colorectal journal in the world

A**l Fistulas in which the Internal opening in not locatable has highest risk of recurrence of fistula. There is no treatment guidelines for such fistulas in the literature.

A simple new protocol to manage such fistulas with high success rate has been formulated and published by us at Garg Fistula Research Institute, Panchkula, India

This is the first paper in Medical literature on treatment of such difficult fistulas
