** RECURRENT INCISIONAL HERNIA **

Recently I had the opportunity to operate on a Recurrent Incisional Hernia, at the site of previous Open Appendicectomy scar (Open Appendicectomy was done at an outside hospital 3 years ago, no documents or operative details available). Patient later developed hernia at the incision site. She underwent Open mesh hernioplasty at another outside hospital 2 ½ years ago. However, she again developed Recurrence at the same site 6 months post surgery.

She now presented to me for revision surgery. Patient was planned for Laparoscopic TAPP PLUS; with Laparoscopic IPOM PLUS as backup if the tissue planes around the site of previous mesh had created too much scaring that prevented the raising of peritoneal flaps. Intraoperatively Peritoneal flaps were raised but due to dense scarring it was not possible to raise peritoneal flaps. T.A.P.P. PLUS was abandoned and on table decision was taken to proceed with I.P.O.M. PLUS. Patient did well in the post-operative period and discharged on P.O.D.-2.

Recently I had the opportunity to operate on a Recurrent Right Side Inguinal Hernia. Patient had earlier undergone Open Mesh repair 4-5 years ago. Laparoscopic T.A.P.P. was done. Patient discharged on P.O.D.-2.

Patient, Mrs. Kela Devi, presented with complains of severe pain abdomen and vomiting for 3 days. She was a known case of gallstone disease for past 1 year. She was examined by Dr. Kumar Sonal (Consultant General and Advance Laparoscopic Surgeon) and recommend admission. All relevant investigations were done and a diagnosis of Acute Calculus Cholecystitis with Gallstone pancreatitis was made. She was started on medical management and stabilised. Surgery (Laparoscopic Cholecystectomy under General Anaesthesia) was done successfully by Dr. Kumar Sonal. Patient recovered well and now discharged.
Get surgery for Gallstone done on time before it becomes a menace.

Another Happy Patient!

Patient name, Master UMAR, 10 years male, was referred to our hospital after R.T.A. with polytrauma with hemoperitoneum. He was examined by Dr. Kumar Sonal (Consultant General and Advance Laparoscopic Surgeon) and planned for Emergency Life Saving Exploratory Laparotomy, for which his relatives consented. Laparotomy and SPLENECTOMY was done. Patient recovered well after surgery and all necessary vaccination were administered for OPSI prophylaxis. Happy patient and relatives on follow-up visit.

As a surgeon there are only a few things that can match the adrenaline rush of performing an emergency life saving splenectomy in a polytrauma case with AAST Grade 4 Splenic injury with hemoperitoneum.
Patient discharged on POD-7 after post-splenectomy vaccination.
