I have done some crazy cases during the course of my post graduation . This one is an endo implant . There was a horizontal fracture due to trauma and we removed the apical segment . I had gotten a customised titanium implant made for the purpose using the required diameter already . After removal of the apical segment surgically , we placed the implant and tried to cement it with MTA . Then placed bone graft . This radiograph is a 10 year recall !! I called the patient to my clinic after all this while for follow up . Shocked as he was to get a call from me , he thankfully came all the way from his hometown . Current radiograph shows a lot of radiolucency and looks like the MTA gave way too . But the tooth is in place . No mobility at all . I asked the patient if he wants to get this removed and get a proper implant as I want to make sure there is no further bone loss now here on forth . He is declined my offer . He wants to go with this one as long as it goes and has promised to come for regular recalls . Let’s see how this goes 🙈

Patient came in with pain with the previously treated tooth . It had secondary caries under the crown , missed canal , ledges and to add to the mix some resorption as well . She is now completely pain free and will be under observation every 3-4 months . She was happy with her life like crown for the tooth as well .

This made my day 😄

We have a temporary solution for the patient who wanted immediate replacement of the tooth . She did not want a removable partial denture , was here only for a single appointment . Luckily we had evertick to the rescue and we could help her . She is happy with it , that’s all that matters for the time being .

This case we saw today morning is primarily diagnosed as “Geographic tongue “or benign migratory glossitis is generally harmless condition which is sometimes genetically predisposed exacerbated by stress . Usually goes away on its own .

Bleaching is a treatment procedure that is done for lightening of the shade of the teeth . An in office procedure is hassle free and risk free . Please do not confuse this with cleaning of teeth where tarter and debris around the teeth is removed .

This patient was told by another practice that after the root canal she needs a cap and there is no other alternative , he even scared her about bleaching saying that it will damage her tooth forever . As doctors it is our responsibility to not propagate false information . She got her root canal done there unfortunately and a metal post was unnecessarily put in the canal . Did non vital bleaching for her . When considerable lightening occurred we matched up a composite filling for the class IV fracture .

Wisdom teeth are most often extracted . But in certain cases they are the only molars present and if the condition is favourable we try to save it and rehabilitation happens using the wisdom tooth . This one was challenging one . Four canals found , cleaned and filled .
