1 week post operative results for a squint surgery in a young woman.
This was an interesting case where a lactating mother who had decompensation of the strabismus wanted a repair.
We waited till the child was on a top feed, ensured there was expressed milk to feed the baby to tackle post operative anesthesia drug wash out before the mother could start feeding.
Promoting breast feeding all the way. With only a back up as needed.
She has achieved binocular single vision, 3D vision and she is perfectly aligned with great cosmesis after surgery.

The weekend that went by: POSN 5, Vadodara
This is pediatric ophthalmology, Strabismus and neuroophthalmology meeting which an academic feast and therapeutic with meeting our friends, mentors, seniors and colleagues.
Perfect space to discuss all the difficult cases. Learning and growing.
My academic contributions:
1. Paper on a study that we conducted and is on going on "peripheral retinoscopy" : this is one of the latest interest in the understanding of myopia progression in children.
2. An e poster: on a complex case of double seronegative ocular myasthenia gravis in a young make
3. An e-video presentation: on squint surgery after LASIK in a middle aged woman
But I learnt far more. We have an amazing set of pediatric ophthalmologists in our country and it was an absolute pleasure to hear Dr Lionel Kowal from Australia as well.

There are some times when I feel it was all worthwhile.
It is this joy of of improving the quality of life of a person- I'm finding it rather hard to describe it in words.
This is one such story of a 6 year old young boy from rural India. You see how he came with this strained neck position. He lived with it for the last 6 years since they didn't believe it could be corrected. Fortunately his aunt brought him to Mumbai & the collaborative efforts and referral by my long time friend that we see this story in a positive light.
This child needed the following;
1. Glasses to see better
2. Surgical correction of - squint, head posture and eye movements
3. Management of lazy eye
He was operated and given glasses and they went back to the village. It was satisfying seeing the immediate gratifying results of head posture and squint correction with a 2line improvement in vision, simply because the eyes were Ina better position to see.
However, lazy eye management is a longer course and needs follow up.
To my surprise, they came back from the village for their first follow up.
Interestingly, many of these challenging cases that need complex surgeries come from backgrounds that cannot afford our standard fee. We do not deny treatment to any patient based on their paying abilities.
We treat them if we can, no matter what.
Here's to believing that the efforts we put in resulted in the family pursuing better vision.
Here's to hoping that his results inspire others in the village, make them aware and persuade them to seek medical attention
Here's to celebrating their courage to go through the journey.
I hope I can put a follow up post with his final visual outcome a couple years down the line....
There are patterns in everything.
And so we see the same in eye movements and squints.
As a squint specialist, we don't only aim to correct what is cosmetically visible to the patient, but we also look at any patterns, tilts In the eye that may need correction.
Hence it needs a meticulous assessment and planning.
And with patterns comes math. So it is all a mathematical equation that we try and solve, knowing well that it is finally human body which may or may not follow the equation to the tee.
So we try our level best based on the evidence that we currently have to correct everything possible. Assessing the risk and benefit of interventions.
In the first image if you were to draw an imaginary line (dotted) extended beyond the photo- it appears like the letter A.
Hence this is called an A pattern squint.

As a doctor We get invested in the patient -emotional more than time.
Some kind words, a beautiful hand written note brings the warmest feeling in return..
This was from a patient I recently operated for
Sometimes even if we get great post op results- we may need the patients to follow up for post operative long term success.
We may prescribe some glasses/ exercises/ patching therapy.
Do not forget to regularly follow up after a great surgical outcome.
The joy of seeing these children:
Early Detection --> early cure
Our Journey to restore binocular single vision
These children treated in time imply better awareness, better education amongst parents.
Let's all work together to ensure children don't grow up assuming:
squints just run in families
* It's normal for children to have squints and they will get better with time
* One should wait for children to grow up and then manage the squints,
* Squints need correction for cosmesis only.
The smile after the surgery becomes bigger.
This middle aged male decided to get operated after a long standing squint- increased in the angle and duration.
It was his confidence and ability to convince clients he was talking to that was at stake.
Such a pleasure to meet these patients after surgery.
This was such a memorable class. One wherein I wondered if they were only residents in training. They were all so well read and answering to the point.
What a delight and so so thankful to .nariani for being such a wonderful messiah and doing such a great job.

There's a lot more to understand about why children are getting myopia at young ages and progressing rapidly.

And like all other conferences that NN organizes, it was more than just a typical conference-a perfect amalgamation of talks, case discussions, debates and fun.
The faculty came in all together as super heroes to tackle one main enemy and form a consensus.
I had the privilege to participate as a speaker sharing our experience with the use if high dose atropine to re**rd myopia progression as well as a panelist.
It was a pleasure to attend the workshops- learn about the open field ARR, peripheral retinoscopy and ortho k lenses.
Im delighted to be back after relishing an extremely delicious and sumptuous 5 course meal on childhood myopia.
Looking forward to bringing MyoCon learnings to clinic.
