We were very sad to remove this tooth for a patient 😔😔
No caries and absolutely well maintained oral hygiene but we are here extracting it with a heavy heart.
Pt came with the complaint of slightly abnormal feeling in the gingival of 46 (lower right first molar). On probing buccal gingival, the probe went on for 10mm indicative of a definite problem in the tooth. No caries, no TOP, no plaque and calculus yet the severe probing depth.
IOPA reveals classic J-shaped lesion guiding the diagnosis towards vertical tooth fracture. On extracting history from the pt, she revealed the history of trauma in the region. Buccal and lingual segments were separated but intact.
So we headed with the extraction of the tooth and during extraction we could see 3 segments of the tooth. And hence the diagnosis proved!!
