मुख के कैंसर में लोअर लिप के भाग का ऑपरेशन और उसका निर्माण कठिन होता है जिसके लिए पांव की हड्डी से जबड़े के कटे हुए हिस्से को बनाया जाता है|
इस प्रकार के कैंसर के लिए मुक्यतः तम्बाकू के साथ चुना के प्रयोग के कारण हो जाता है | इस तरह के कैंसर के मरीज का सफल ऑपरेशन हुआ है और अभी मरीज अस्पताल से डिस्चार्ज भी होने वाला है | इस सफल ऑपरेशन के लिए पूरी टीम को धन्यवाद् |

शिशु को जन्म देने के बाद महिलाओं के कान में सिटी का बजना और कम सुनाई देने की समस्या प्रारंभ हो सकती जिसको ओतोस्क्लेरोसिस (otosclerosis) कहते हैं | इस बीमारी में कान की हड्डी में अप्रत्यषित तरीके से हड्डियों का जमना शुरू हो जाता है | इस बीमारी का इलाज़ ऑपरेशन होता है और ज्यादा देरी हो जाने के बाद कान की मशीने से ही इलाज़ संभव है | कान की इस तरह की समस्यायों के लिए तुरंत मिलिए ..........

65 yr old female with malignant thyroid growth operated as total thyroidectomy with central plus left lateral neck dissection. Patient recovered sussefully with hpe papillary carcinoma thyroid. Now referred to nuclear med deptt for further management. All credit goes to our surgical team, dr atul, abhishek,alok, vivek and a very competent anaesthetist.

Lipoma of size greater than 10 cm and in anterior part of neck considered giant and rarest locations in head and neck region. Lucky to operate on two patient with these presentations.....

65 yr old patient with TYPE II DM and HTN developed right sided cervicofascial abcess and aggressive cellulitis and septicemia after right upper molar extraction in a remote area. patient underwent emergency surgical exploration and drainage ( trans oral and transcervical route ) last week. After good antibiotic coverage and regular dressing ,patient improved and discahrged with oral med . thanks to dr Vineet Agrawal (very competent anesthetist), dr Karmraj sir for providing me a platform & support and a good team having alok and santosh with others to save this patient.

Stapes- smallest bone in our body in ear and when get involved in otosclerosis causes major discomfort to patient. Did successful stapedotomy with Teflon piston in young patient .

A 12 yr old girl with primary cholesteatoma and post auricular fistula operated yesterday. Huge cholesteatoma sac with fistula in horizontal canal, total ossicular erosion, tegmen defect present. Two fistulous opening from mastoid bone present, one near to mastoid apex and another on posterior surface. Modified radical mastoidectomy with wide meatoplasty with fistulous tract excision done without tympanoplasty. She is recovering well .....

12yr old boy with right nasal cavity mass with limited extension excised intoto with its stump from nasal septum also excised and cauterised. HPE awaited......

Ear surgery started on regular basis at central railway hospital BLW, VARANASI after brief interruption of covid. Labomed 5 step with digital camera operating microscope installed.

70 yr male with neg rtpcr developed mucormycosis with left eye vision loss to finger counting. KCO DM with steroid in covid.Urgent debridement done via denkers approach with orbital clearance. Video is not good quality per se and unedited. Patient is recovering .......

Another interesting case operated today.......
A 30 yr old male came with huge left side neck swelling with firm consistency of size 10 cm by 7 cm. swelling was present for than 15 yrs and increased to present size gradually. Cect neck suggestive of benign tumour with complex cystic consistency with eccentric calcification. Swelling was abutting carotid and IJV on left side with compressed left thyroid gland. fnac not done as patient not willing. Patient operated with transcervical approach with delivery of neck mass intoto. Hpe report awaited.

I request every parents whose children is less than 5 years old to have hearing screening of their children as soon as possible. It may cause a great impact on your children in future ( if they have some hearing loss unknowingly). I request every clinicians to educate others regarding same and have otoacoustic emission (OAE) screening for new born within three months.

Submandibular gland excision
3:2:1 means 3 nerves ie marginal mandibular, hypoglossal and lingual nerve. 2 vessels facial artery and vein and last and most imp 1 duct - wartsons submandibular duct which should be double ligated.

Another dcr right side with good exposure of lacrimal sac. Even we can see inside of it with endoscope. Very short video here

A type 3 MND performed for right side with primary tongue malignant lesion at right lateral border of size >1.5 cm. Although planned for SOHND, but proceeded with functional neck dissection No Neck nodes on radiology but found many at level 2. spinal accessory nerve visualized with other terminal branches. primary tumor removed without any flap reconstruction. HPE AWAITED.

TAKE CARE OF YOUR TONGUE -
When u look at this picture, its shows a very good drainage of tongue lymphatic system. it also signifies the importance of any lump or swelling or mass related to tongue should never be avoided. Any mass or malignant lesion of tongue has high chances of metastases if avoided and should be treated early.

BELL'S PALSY- TIME IS THE KEY
In 8 Months, 18 patients visited with this, 15 recovered within 20 days , however, 3 patients took extra time. Two recovered fully in 3 months while other being pregnant still recovering but slowly.

growth right pinna - removal with ssg - follow up- good uptake. Surgery done in 2017.
