I was honoured to be invited to participate in this very well organised “Tabuk International Surgery Conference: Updates in Surgical Management of Cancer” (26-28 September 2022) with two talks:
1- Laparoscopic D2 Gastric Surgery
2- Laparoscopic Pancreatic Resections: can it be justified?

It is most gratifying to receive such feedback from patients, particularly after advanced laparoscopic surgery. This young girl did very well following laparoscopic distal pancreatectomy with preservation of the spleen (Warshaw technique) for a solid pseudopapillary tumor of the body of the pancreas and was discharged on the 3rd postoperative day. This is the 10th consecutive laparoscopic pancreatic resection procedure at Burjeel Hospital, Abu Dhabi since I joined UAE 2 years ago. I am grateful for the opportunity to offer my patients the advantages of laparoscopic surgery.

My brief contribution to this debate.

We discuss with a UAE medical expert the scenarios that make one a better choice over the other.

Glad to report our 5th and consecutive laparoscopic Whipple’s pancreaticoduodenectomy at Burjeel Hospital, Abu Dhabi, the only center in UAE currently offering this type of complex surgery laparoscopically and successfully with sound oncologic outcomes and to the benefits of our patients.

This 56-year-old British woman presented with obstructive jaundice secondary to signet-cell carcinoma on a background history of Crohn’s disease for which she previously underwent two open ileocolic resections and a subsequent mesh repair of incisional hernia; the residual distal small bowel structures and dilatation were successfully managed with monoclonal antibodies.

The surgery was uneventful, drains removed on day-2, and she was discharged home on day-4 (picture taken in clinic on day-10). The histology showed an R0 resection of a 5x4x1cm signet-cell duodenal carcinoma, G3, pT3aN0 (0/17 LNs), PNI-, LVI-.

I’d like to extend my thanks and appreciation to the multidisciplinary teams and Burjeel Hospital and Burjeel Medical City, to the amazing theatre staff, and the nursing staff for their care.

Pleased to report what we believe to the best of our knowledge to be the first Laparoscopic right hemihepatectomy in UAE. This 48-year-old British woman had an uneventful intraoperative and postoperative course (100ml blood loss, 20 minutes Pringle manoeuvre), needed no blood transfusion and was discharged on the 4th postoperative day. The liver resection followed 6 cycles of chemotherapy after a laparoscopic left hemicolectomy for 3 synchronous liver metastases, one of which was situated at the inflow to the right hemiliver. Histology confirmed a complete resection (R0) with the nearest margin 7.5mm away. My gratitude goes to all the theatre staff, the members of the MDT team at Burjeel Hospital, Abu Dhabi and Burjeel Medical City, the radiologist who carried out an accurate liver volumetry, and the nursing staff in theatre, ICU and wards for their care and support.

Hepatic hydatid cysts could mimic biliary cystadenomas and vice versa. Although the MRI scan on this 27-year-old man with a 12.5x7.5cm multiseptated cyst favoured the latter, hydatid serology was positive.

Following a 4-week course of albendazole, he underwent a laparoscopic left hepatic lobectomy (segments II & III with extension into IVb). The cyst was adherent to stomach and pancreas. He made uneventful recovery and was discharged on the 3rd postoperative day.

Histology confirmed a hydatid cyst.

Laparoscopic transgastric Pancreatic pseudocyst-gastrostomy with necrosectomy and cholecystectomy is an effective minimally invasive approach.

This patient made an uneventful recovery and was discharged on the 3rd postoperative day (photo at follow up a week later).

This compliments our previously published experience with the various methodological approaches to laparoscopic internal drainage of pancreatic pseudocyst. https://pubmed.ncbi.nlm.nih.gov/19789929/

Pleased to report our fourth and consecutive laparoscopic Whipple’s procedure at Burjeel Hospital, Abu Dhabi for a 3cm neuroendocrine tumor in the head of pancreas (pT2N0, 0/14 LNs).

The soft pancreas with a 2mm pancreatic duct was invaginated into the jejunum using a modification of Blumgart’s technique. Drain removed on day-3 (no POPF) and patient discharged on day-5.

Laparoscopic splenectomy with preservation of the accessory spleen for a 9.0cm symptomatic littoral cell angioma.

This is a very rare primary splenic hemangioma that originates from the red pulp of the spleen and carries a malignant potential. It is often associated with other visceral malignancy.

Spleen removed intact through a Pfannenstiel incision. Operating time 93min. Patient discharged on day-1.

Pleased with our fourth and successful experience with this laparoscopic vagus-sparing proximal gastrectomy and intracorporeally-sutured oesophago-gastric anastomosis.

This 50-year-old man had an 11cm GIST of the cardia of the stomach that reduced to 6cm after 3 months of Imatinib. Preservation of the body and tail of the pancreas and the left adrenal gland was possible with painstakingly fine dissection of the closely adherent tumor.

Surgery was rewarded with compete resection (R0) and rapid recovery as the patient was discharged on the third postoperative day and did well at follow up 10 days later.

This 28-year-old maintenace engineer and I are very pleased with his swift recovery after laparoscopic distal pancreatectomy with en bloc resection of the splenic vessels and preservation of the spleen (Warshaw technique) for a 6.5cm solid paeudopapillary tumor.
