ATHLETIC PUBALGIA
Groin pain/ Athletic pubalgia is a common entity in athletes
- Sports that require specific use (or overuse) of lower abdominal muscles and the proximal musculature of the thigh
- Ice hockey, football, basketball...
Athletes usually present with:
- Exercise-related unilateral lower abdomen and anterior groin pain
- Deep, sharp pain that can radiate to the proximal thigh, low back, lower abdominal muscles, perineum or sc***um
Clinically, positive tenderness with palpation, contraction and stretching confirm the diagnosis.
Imaging studies such as US, MRI and CT can help with the diagnosis.
Conservative treatment should be attempted for 3 months before considering surgery.

Let's talk about HIPS!
FAI = Femoroacetabular (Hip) impingement occurs when the femoral head (ball of the hip) pinches up against the acetabulum (cup of the hip).
This can cause damage to the cartilage that surrounds the acetabulum called labrum, causing hip stiffness and pain in the groin or front thigh (especially when bending over) and can lead to arthritis.
The location of the deformity in the joint (the femoral head or acetabulum) defines the two main types of FAI.
The diagnosis is made after thorough history, physical exam and specific imaging.
Figure A: Impingement test = FADIR test = Forced flexion, internal rotation, and adduction of the involved extremity reproduces symptoms related to femoroacetabular impingement.
Figure B: Posterior impingement test = FABER test = Pain is reproduced posteriorly in patients with involvement of the posterior acetabulum as the extremity is forcefully externally rotated.
Nonoperative and surgical treatment options are available to treat hip impingement.

Let’s talk about SHOULDERS!
A SLAP tear or SLAP lesion is an injury to the glenoid labrum.
It is one of the most common injuries seen in throwing athletes causing what is referred to as the “Throwing shoulder”.
Symptoms:
- shoulder popping, locking, or grinding
- pain with certain movements or positions
- pain when lifting things, especially over head
- reduced range of motion
- shoulder weakness
Diagnosis:
SLAP lesions are difficult to diagnose as they are very similar to those of instability and rotator cuff disorders.
The SLAP diagnosis appears to be a clinical impression. MRI is the most common imaging tool used to diagnose labral lesions. An MR arthrogram is also used.
A total of FOUR types of superior labral lesions involving the biceps anchor have been identified.
Treatment:
The surgical treatment of a SLAP lesion becomes more and more frequent as the surgical techniques, the implants and the postoperative rehabilitation of the patient are improved and provide in most cases an excellent outcome.
- SLAP I lesions are treated with debridement.
- SLAP II lesions, whose therapy is either fixation of the superior labrum or tenotomy/tenodesis of the long head of the biceps tendon, are the most controversial.
- For patients with SLAP III or IV lesions the most commonly accepted approach is tenotomy or tenodesis of the long head of biceps tendon.
I see many of my teammates using various methods of taping to keep from re-spraining their ankles. Does ankle taping really work?
Taping is a common way to support a joint, especially the ankle. The goal is to reduce the risk of primary injury and avoid another ankle sprain. But does it work? And if it does work, what's the mechanism behind the effect?
The general consensus is that taping has a preventive role. There are two most likely explanations for this.
First, tape gives the joint mechanical support. The tape keeps the ankle joint from moving too far in any one direction.
Second, it enhances proprioception. Proprioception allows the ankle to feel changes in the surface or slope of a surface. That way the ankle and foot can make necessary corrections to stay steady and balanced while walking or running.
More research concerning this topic are needed in order to verify the real role of taping, comparing with placebo.
All things considered, it's still a good idea to use taping to prevent spraining the ankle.
