PHYSIOTHERAPISTS use STETHOSCOPE ?
I find that many people do not know that Physiotherapists (PT’s) work in hospitals, when in fact there is a Physiotherapist assigned to almost every floor/ unit throughout a hospital. Physio in a hospital looks very different from what you may have experienced or imagined when you go to a clinic, or what’s called outpatient/ private practice settings.
No I’m not “massaging” anyone
No I’m not teaching people how to run
No I’m not applying electric machines to patients
As a physiotherapist working in the ICU (intensive care unit): .
I am mobilizing patients who can’t themselves
I am treating patients chests/ lungs who are drowning in secretions with manual techniques
I am suctioning patients with tracheostomy’s and on ventilators
I am moving people who have 3-10 different lines connected to them for their medical stability
I am working with nurses, OTs, RT’s, Social workers, and Drs to provide appropriate treatment
I am listening to patients lungs ( with my stethoscope) to assess their airways
I am administering oxygen when needed
I am manually moving patients joints to prevent contractures
I am getting people on ventilators sitting, standing, and mobile
I am improving patients quality of life
I am helping people get better. .
These are only some of the responsibilities of a cardiorespiratory physiotherapist working in acute care settings, and what I do is different from what other physiotherapists in hospitals are doing!

🔺️ What is Bell's Palsy?
♠️ Bell's palsy, also known as idiopathic facial palsy, is a form of temporary facial paralysis or weakness on one side of the face.
♠️ It results from dysfunction of cranial nerve VII (facial nerve) which directs the muscles on one side of the face, including those that control eye blinking and closing and facial expressions such as smiling.
♠️The facial nerve also carries nerve impulses to the tear glands, the salivary glands, and the muscles of a small bone in the middle ear.
♠️ The facial nerve also transmits taste sensations from the tongue.
🔺️ How is it treated?
♠️ For individuals with new-onset Bell’s palsy, steroids are highly likely to be effective and can increase the probability of recovery of facial nerve function.
♠️ In most instances, oral steroids should be started within 72 hours of symptom onset if possible, to increase the probability of good facial functional recovery

♠️ Observation :
✳ Patients with piriformis syndrome may present with gluteal atrophy and shortening of the limb on the affected side (LLD)
✳ In chronic cases, muscle hypotrophy present in the affected extremity
♠️ Assessment :
The physical assessment should include the following,
♦️ an osteopathic structural examination with special attention to the lumbar spine, pelvis and sacrum and look for any leg length discrepancies
♦️ diagnostic tests
♦️ deep-tendon reflex testing, strength and sensory testing
♠️ Palpation :
💧 The patient reports tenderness on palpation at the greater sciatic notch, in the region of sacroiliac joint or over the piriformis muscle belly

💧 With deep digital palpation in the gluteal and retro-trochanteric area, tenderness and pain may occur with an exacerbation of tightness and leg numbness

🤯 STROKE 🤯
♦️ You may have heard, loss of blood supply occurs in the brain, due to a block/ clot in blood vessels in the brain or due to rupture of blood vessels which can damage the the respective parts of the brain and lead to severe morbitidy or death ⁉️
♠️ Death of the nervous tissue in the brain or the cells of the brain is named as stroke, which may happen in two main ways :
1. ISCHEMIC STROKE -
♦️ This is the most common type.
♦️It is usually caused by a blood clot that blocks or plugs a blood vessel in the brain. This inhibits blood from flowing to the brain. Within minutes, brain cells begin to die
♦️ Another cause is stenosis, or narrowing of the artery
2. HAEMORRHAGIC STROKE -
♦️ when blood from an artery begins bleeding into the brain
♦️ This happens when a weakened blood vessel bursts and bleeds into the surrounding brain
♦️Pressure from the leaked blood damages brains cells and as a result the damaged area is unable to function properly
🎯 These scenarios can lead to improper function of the one or more parts of the brain and movement paralysis of the limbs
✳ Signs and Symptoms :
♠️ Complete or partial loss of movement in one side of the body (Hemiplegia)
♠️ Improper speech
♠️ Dizziness
♠️ Reduced perception
♠️ loss of vision (Unilaterally)
♠️ There can be a terrible headache with a ischemic stroke
✴ These symptoms will appear as soon as the brain cells die
✴ If these symptoms try to resolve within 24 hours from the onset of symptoms, it is known as TRANSIENT ISCHEMIC ATTACK (TIA) , is a normal condition, which may lead to a severe stroke in the near future
💧 Proper medical advice should be taken after a TIA, to avoid any bad consequences

👌 It is an important responsibilty of Physiotherapists to maintain the body mechanics and muscle strength of a patient suffered with stroke during the stay at ICU and to make them independent as much as possible after discharge from the hospital
⏩ The stroke patient can be made independent in his/her daily life activities by various physiotherapy treatment techniques namely borbath techniques, proprioceptive techniques, motor learning skills, hydrotherapy etc., with proper assessment and examination
⏩ Movement paralysis of arms, legs , face and various movement patterns can be made normal using physiotherapy techniques
👍 The patient who gets a disability is not supposed to be in the same condition for lifelong rather proper and early treatment procedures can help, lead an independent life 😍
🎯 BAMBOO SPINE
⏩ Bamboo spine is a radiographic feature seen in a condition called ankylosing spondylitis that occurs as a result of vertebral body fusion by marginal syndesmophytes*.
⏩ It is often accompanied by fusion of the posterior vertebral elements too
♦️ A bamboo spine typically involves, the thoracolumbar and/or lumbosacral junctions and predisposes to unstable vertebral fractures and Andersson lesions
♦️ In a bamboo spine, the outer fibers of the annulus fibrosus of the intervertebral discs ossify, which results in the formation of marginal syndesmophytes between adjoining vertebral bodies
♦️ The resulting radiographic appearance, is thin, curved, radiopaque spicules that completely bridge adjoining vertebral bodies.
♠️ There is also accompanying squaring of the anterior vertebral body margins with associated reactive sclerosis of the vertebral body margins (shiny corner sign)
✳ Together, these give the impression of undulating continuous lateral spinal borders on Antero posterior spinal radiographs and resemble a bamboo stem; hence the term bamboo spine

Yale launches clinical trial for drug to treat severe COVID-19 patients ✔✔✔
♦️ Yale researchers will begin a clinical trial at Yale New Haven Hospital to test the effectiveness of a drug called ibudilast (MN-166) for treating acute respiratory distress syndrome (ARDS) which is a life-threatening lung condition developed by some of the most seriously ill COVID-19 patients
♦️ The drug , which has been approved for years in Japan and Korea for the treatment of asthma - has shown promising results for reducing inflammation associated with ARDS in mouse models
♦️ Ibudilast is a type of drug known as a MIF inhibitor
♠️ MIF is a gene that regulates the immune response - a driver of inflammation and the so-called “cytokine storm”
♠️ Over-expression of the gene has been found to play a key role in a number of diseases including ARDS, asthma, rheumatoid arthritis, lupus and multiple sclerosis

✳ Osteoarthritis occurs when the protective cartilage that cushions the ends of the long bones wears down over time due to improper weight bearing or excessive weight
✳ The space between the ends of bones reduces and frictions over each other while doing movement and weakens the surrounding muscles
✳ Although osteoarthritis can damage any joint, the disorder most commonly affects joints in the hands, knees, hips and spine
✳ A chronic condition can affect the hip or ankle joints simulatneously
😳 Risk Factors :
♦️ Excessive body weight
♦️ Trauma
♦️ Altered Hamstring: Quadriceps (H/Q) Ratio
♦️ Age (morethan 45 years old, after menopause)
♦️ Women (Males get too with higher incidence in females)
👨⚕️ Examination :
✴ Xray is taken for the affected joint to look for the space reduction between the joints and formation of osteophytes
✴ Crepitus can be felt over the joint during the joint movement
✴ Pain while climbing the stairs and sitting on a lower couch
🎲 When the joint gets damaged severly causing terrible pain while weight bearing needs to undergo a Surgery named as TOTAL KNEE REPLACEMENT (TKR)
PHYSIOTHERAPY MEASURES TO BE UNDERTAKEN :
💧 Strengthening the surrounding muscles using isometrics or resistance
💧 Joint manipulation techniques
💧 Electrotherapy
💧 Modification of lifestyle according to proper physical therapy advices (weight bearing, avoid climbing stairs or squatting)
