Heavy Menstrual Bleeding (HMB) is a common and abnormal type of uterine bleeding resulting in excessive and prolonged blood loss.

What constitutes normal periods?
Normal periods or me**es last for 2-7 days, occur at cycles of every 21-35 days and blood loss should not exceed 80mls (equivalent to 16 teaspoons of blood).

Do your periods last longer than a week?
Do you soak through one pad/tampon every hour for several consecutive hours?
Do you flood through your clothes or bedsheets?
Do you use two types of sanitary products simultaneously?
Do you pass blood clots larger than 2.5cm?
Do you need to change your pads/tampons during the night?

If you answered yes to these, consult your Gynae.

One might feel no symptoms whatsoever or you may complain of fatigue, lack of energy, shortness of breath, palpitations, dizziness and chest pain – these are all symptoms of iron deficiency anaemia that results from HMB. It can therefore be very disruptive to one’s daily life and needs to be tackled.

HMB can be due to a variety of conditions and structural causes including
- Polyps (benign growths in the lining of the uterus or cervix)
-Fibroids (benign tumours that occur within the muscle layer of the uterus)
-Endometriosis (a condition where the uterine lining can be found in other locations such as the ovaries - this can cause heavy and painful periods)
-Adenomyosis (here the glands in the uterine lining become embedded within the uterine muscle, this can give rise to painful and heavy me**es)
-Cancer of the uterus or the cervix

Other conditions include:
-Inherited blood clotting disorders
-Ovulatory disorders (this is due to unopposed oestrogen on the uterine lining causing it to thicken and resulting in HMB.
-Medications (including hormonal preparations, anticoagulants and the copper intrauterine device)
-Pelvic inflammatory disease (an infection of the female reproductive organisms)
-Pregnancy related (e.g. miscarriages)

The ultimate aim here is that of improving one’s quality of life.

Medical options include non-hormonal options:
-Tranexamic acid (tablets which are started at the beginning of your period, these help the blood to clot)
-Nonsteroidal anti-inflammatory drugs (e.g. mefenamic acid)

Hormonal options include:
-The levonorgestrel intrauterine system (this is a device that is placed inside the uterus and gradually releases progesterone to the inside of the uterus causing the uterine lining to become thinner and hence decreases blood loss)
-The combined oral contraceptive pill
-Cyclical oral progesterone

Surgical procedures include:
-Operative hysteroscopy (this removes polyps and certain types of fibroids)
-Uterine artery embolization (a procedure that is used to shrink fibroids by occluding the blood vessel that supplies them)
-Myomectomy (surgical removal of fibroids)
-Hysterectomy (removal of the uterus, this is usually performed as a last resort after other treatment options have failed).
