Throwback to ENT screening session that was conducted on 19/08/2024 at IPD Kluang. ENT (ear, nose & throat) screening was offered to the public who wanted consultation for ENT related problems.

The session was conducted successfully with good response and feedback from Kluang IPD staff and family members. This event allowed the public to enquire regarding ENT diseases and to answer questions on ENT conditions that they have. Screening participants were counselled on various ENT problems that were encountered, options of treatment and further management options that can be done.

Throwback to ENT screening session that was conducted on 21/05/2024 at IPD Batu Pahat. The ENT (ear, nose & throat) screening session went well and was conducted successfully thanks to the attendance of the IPD staff in taking part in this event. Hidung, telinga dan kerongkong adalah saling berkait antara satu sama lain. Oleh itu, pemeriksaan menyeluruh semua 3 lokasi perlu untuk diagnosis & rawatan yang tepat.

Faktor risiko kanser pangkal hidung : Sejarah kanser dalam keluarga, faktor pemakanan, tabiat merokok, terdedah kepada habuk kayu

Saringan awal kanser pangkal hidung - Umur>40, Sejarah kanser dalam keluarga, gejala hidung/tekak/telinga, kehadiran faktor risiko

Hidung berdarah? Hidung sumbat? Bengkak kelenjar leher? Kurang pendengaran? Semua ini adalah petanda kanser pangkal hidung.

NASOPHARYNGEAL CARCINOMA (NPC)

Nasopharyngeal cancer (also known as Nasopharyngeal carcinoma or NPC) is a type of head and neck cancer. NPC is a cancer involving the nasopharynx, an area located behind the nasal cavity (posterior nasal cavity) above the soft palate, which provides air passage between nose and the throat.
The most common site of this cancer in the nasopharynx is called Fossa of Rosenmuller (FOR) as shown in the pic.

Epidemiology
• 5th most common cancer in Malaysian population (4.0% of total cancers)
• 4597 cases from 2012 – 2016 (73.1% males, 26.9% females)
• 5th most common in males (6.5% of total male cancers) in Malaysia after colorectal, lung, prostate and lymphoma cancers
• Ethnic breakdown (Age standardized incidence rate, ASR)
o Chinese men – 8.6 per 100,000
o Chinese women – 2.9 per 100,000
o Malay men – 2.7 per 100,000
o Indian men – 0.6 per 100,000

• Johor – 2nd highest – 658 cases (2012 – 2016)
o Men – 6.0 per 100,000
o Women – 2.4 per 100,000

Risk Factors
• Genetic
o Family history of cancer (especially NPC)
o Chinese ethnicity and natives of Sabah & Sarawak

• Gender – Male to female ratio = 3:1
• Dietary factors – Consumption of salted fish & preserved vegetables (nitrosamine content)
• Lifestyle & Environment
o To***co smoking
o Exposure to wood cooking fire
o Exposure to occupational solvents
o Occupational exposure to wood dust

• Epstein-Barr Virus infection

Clinical Presentation
• Painless neck swelling (unilateral/bilateral)
• Blood stained nasal discharge/saliva with/without nasal block
• Unilateral ear block or hearing loss
• Headache
• Blurring of vision/Double vision (Diplopia)
• Facial numbness

Patterns of NPC spread
Complications and new symptoms can arise as a result of the cancer spreading at its local site. Patterns of spread and its symptoms are illustrated in the pic below.

Investigation
Suspicion of this cancer will require further investigation to confirm this diagnosis which are described below :
• Nasopharyngeal examination via endoscopy and biopsy
• Blood Ix
• Imaging – CT scan

Nasopharyngeal cancer is one of the most common cancers affecting Malaysians. Its silent nature in onset makes it difficult to detect early and symptoms usually present at advanced stage in most cases.

However, recognition of risk factors and early screening enables detection in its early stages. In addition, early detection and good response to chemoradiotherapy treatment increases survival rates and reduces impairment to affected areas.
