Hello, I am Dr. Jun from 👩‍⚕️, and I would like to share a short story not only about an elderly patient I attended to but also about the silently struggling caretaker, who happens to be the patient’s daughter.

I was assigned to visit Madam Liew, a 78-year-old lady who had suffered a major stroke and was currently bedridden, relying on a feeding tube for nutrition. Her only caretaker is her daughter👩, who is a single mother of 3👦🧒, working a 9 to 5 job, and is always running here and there, squeezing time out to care for her sick and bedridden mother.

Madam Liew’s feeding tube was dislodged in the morning, likely pulled out by herself, as many other patients find it uncomfortable. This is a case our homecare team commonly encounters. We have a team of doctors and nurses available via a hotline☎️, and within 2 hours, we are at your doorsteps🏠, ready to solve your problem😉.

So, I was at Madam Liew’s house, welcomed by her tired-looking and flustered daughter😢, who had made multiple phone calls to various clinics, trying to find a doctor or nurse to come to the house to re-insert the feeding tube, but to no avail. She had also taken an emergency leave from work, settled her kids at school, and was finally referred to by one of her colleagues.

I proceeded with my examination of Mdm Liew, followed by the requested medical procedure🩺, and after 30 minutes, Mdm Liew had a new silicone feeding tube in place and was ready to feed again.

"Aunty, you must be hungry already. No milk since morning! After this, don't pull out the tube anymore, okay?😃" I said to Mdm Liew, to which she gave a tiny cheeky smile☺️💕.

Before leaving the house, I spent a few minutes with the daughter. She told me a long story about how her mom got sick🤒, the frequent hospital visits🏥, the medical expenses💸, medications💊, basically everything and anything about the care of her bedridden elderly mother.

I listened and nodded. She had certainly gone through immense struggles and challenges and was still holding on with strength and courage for the sake of her sick mother and her young children.

I might have opened the floodgates of tears because she started crying😰. "Dr. Jun, you have no idea. It’s really not easy. I always feel like I want to give up, but I cannot!" "𝑻𝒉𝒂𝒏𝒌 𝒚𝒐𝒖 𝒔𝒐, 𝒔𝒐 𝒎𝒖𝒄𝒉 𝒇𝒐𝒓 𝒄𝒐𝒎𝒊𝒏𝒈 𝒕𝒐 𝒔𝒆𝒆 𝒎𝒚 𝒎𝒐𝒕𝒉𝒆𝒓. 𝑰 𝒄𝒂𝒍𝒍𝒆𝒅 𝒖𝒑 𝒔𝒐 𝒎𝒂𝒏𝒚 𝒄𝒍𝒊𝒏𝒊𝒄𝒔, 𝒏𝒐𝒏𝒆 𝒐𝒇 𝒕𝒉𝒆𝒎 𝒄𝒐𝒖𝒍𝒅 𝒄𝒐𝒎𝒆."

All I could offer at that moment was a moment of silence, followed by a gentle pat on her shoulder and some words of comfort and encouragement. But I am glad that today, our homecare team can provide daily support to Mdm Liew and her daughter✨. With daily visits from our nurse, Mdm Liew is receiving professional nursing care every day, along with timely feeding tube changes, urine tube changes, and bedsore treatments.

This has definitely lifted a huge burden off her daughter’s shoulders. Mdm Liew is well, receiving professional nursing care daily, and her unsung hero of a daughter can now have an improved quality of life💕

什么是褥疮？
褥疮是由于皮肤长期受压而产生的伤口。长时间不动的人，例如卧床不起或使用轮椅的人，最容易患褥疮。摩擦、潮湿和牵引力（拉扯皮肤）也会导致褥疮。这些疼痛的伤口或压疮可能会变大并导致感染。在某些情况下，褥疮可能会危及生命。

哪些人有患压疮的风险？
老年人、行动不便或卧床不起的人最容易患褥疮。

身体的哪个部位最容易出现压疮？
褥疮最有可能发生在骨骼最靠近皮肤的身体部位，例如：
✅ 脚踝
✅ 背部
✅ 臀部
✅ 肘部
✅ 脚后跟
✅ 臀部
✅ 尾骨

如何预防褥疮？
这些步骤可以降低患褥疮的风险：
1. 如果您坐着，则每 15 分钟更换一次姿势；如果您躺在床上，则每一到两个小时更换一次姿势。如果您无法重新调整自己的位置，看护者可以帮助您做到这一点。
2. 定期检查您的皮肤（或让护理人员检查），观察皮肤颜色和感觉的变化（压痛或疼痛、温暖或凉爽）。
3. 吃有营养的饮食并保持充足的水分。
4. 保持皮肤清洁干燥。使用保湿霜来保护皮肤免受汗水、尿液和粪便的侵害。
5. 使用专门设计的床垫或泡沫垫来减轻皮肤的压力。

What are bedsores?
Bedsores are wounds that occur from prolonged pressure on your skin. People who are immobile for long periods, such as those who are bedridden or use a wheelchair, are most at risk for bedsores. Friction, moisture and traction (pulling on skin) also lead to bedsores. These painful wounds, or pressure ulcers, can grow large and lead to infections. In some instances, bedsores can be life-threatening.

Who are risk for pressure sore?
People who are older, immobile or bedridden are most at risk for bedsores.

Which part of the body tend to develop pressure sore the most?
Bedsores are most likely to develop on the parts of your body where your bones sit closest to your skin, such as your:
✅ Ankles
✅ Back
✅ Buttocks
✅ Elbows
✅ Heels
✅ Hips
✅ Tailbone

How can you prevent bedsores?
These steps can lower your risk of bedsores:
1. Change positions every 15 minutes if you’re seated or every one to two hours if you’re in a bed. A caregiver can help you do this if you’re unable to reposition yourself.
2. Check your skin regularly (or have a caregiver do it) to look for changes in skin color and sensations (tenderness or pain, warmth or coolness).
3. Eat a nutritious diet and stay well hydrated.
4. Keep your skin clean and dry. Apply moisture barrier creams to protect your skin from sweat, urine and stool.
5. Use specially designed mattresses or foam cushions to ease pressure on your skin.
