This was the first time I did 2 presentations on the same day at 2 different locations, Kuala Lumpur and India.

Exactly a week ago I presented and shared my approach on periorbital aesthetics at the WAAARNEM 2022 organised by SAARMM. This is an annual meeting on anti ageing, aesthetics and regenerative medicine where doctors share their experiences and expertise. This was the first physical meeting I attended and presented since 2020.

This was followed by a virtual presentation at the OPAI Midterm conference held in Aurangabad, India. It was exciting to share my view and experience on aesthetic eyelid surgery in East Asians. One of the key points I wanted to highlight was, in my humble opinion, double eyelid surgery should be known as East Asian blepharoplasty and not just Asian blepharoplasty. What’s you thoughts on this 😂😅

Posted • Eyelid revision surgery is complex and painful to both surgeon and patient. It requires great anatomical knowledge and meticulous planning and ex*****on to revise previous failed surgery. Sometimes surgery has to be done in stages, first to revise the complication then refine the final outcome.

This patient was unfortunate to have her eyelid surgery done by untrained practitioner. She had 2 surgeries by the same person but result was worse. She ended up with a high, deep crease and ptosis making her eyelid extremely asymmetrical.

The first stage of the surgery was to lower the crease and ptosis correction. Then the eyelid scar was treated with anti scarring injection before proceeding to enhance the eye by double eyelid adjustment and epicanthoplasty.

It was a hard and frustrating journey for this patient as she has to travel from outstation. But I’m so glad that we finally managed to revise her eyelid and make her look more decent.

Eyelid surgery is delicate. It’s best to avoid complications as eyelid surgery revision can be extremely challenging and frustrating.

To legally practise medicine in Malaysia, medical practitioners are required to be registered with Malaysian Medical Council (MMC).

The principal aim of the MMC is to ensure the highest standards of medical ethics, education and practice, in the interest of patients and public. You can search the name of practitioner if he/ she is registered with MMC via the link below.

This is an important message for those who had filler injections or plan to have filler injections. For many years, we thought Hyaluronic acid (HA) fillers last for 9-12 months or at the most 18 months. However, we now know that it lasts for a much longer time, at least in some individuals. My friend reported that he has seen fillers lasted for more than 12 years.

I operated on this patient yesterday who came for eyebag surgery. I had suspicion that there was HA fillers in her under eye and cheek area and advised to dissolve. However, patient claimed that her filler injections were done in 2014 and had no more fillers after that. She was also adamant that the fillers were long gone since it was 8 years ago and refused to be dissolved.

I proceeded with transconjunctival lower blepharolasty with fat transposition. Lo and behold, this translucent material was visible after I dissected the periosteum (a fascia covering the bone surface) just before the fat transposition. I proceeded with surgical removal of some of the fillers as there was a good chunk of it.

This affirms my belief that HA fillers last much longer than what we were told. I won’t say this applies to everyone, as how HA filler is metabolised in the body differs in different individuals. Another thing I observed was there was a big chunk of fillers during surgery. This also confirms my belief that HA filler expand over time.

Don’t get me wrong, I’m not against HA fillers. In fact, I love HA fillers. What I’m saying is you should not go overboard with HA fillers as it expand with time and last much longer than we think. As aesthetic practitioners, we can reduce and avoid more puffy faces walking around.
