A patient asked me “doc pag naalis ba wisdom tooth ko mawawala din wisdom ko?” (It was a serious question from him 😅😅😅)

📍Why are 3rd molars called Wisdom tooth ?

“Wisdom teeth earned their title because they usually appear between ages 17 and 21, or the time when people used to think wisdom was gained”

But it doesn’t mean that your 3rd molar is directly connected to you wisdom.

𝟏. 𝐓𝐞𝐞𝐭𝐡 𝐦𝐚𝐲 𝐬𝐡𝐢𝐟𝐭 𝐚𝐧𝐝 𝐥𝐨𝐨𝐬𝐞𝐧
Kapag nawala ang katabi ng ngipin, maari itong tumagilid. Ang opposite ngipin naman ay pwedeng umangat. Ang pag-drift ng ngipin ay pwedeng magcause ng bite problems, jaw joint problems (TMJD), gum disease, at possible na matanggal din ang ibang ngipin.

𝟐. 𝐘𝐨𝐮𝐫 𝐟𝐚𝐜𝐞 𝐜𝐨𝐮𝐥𝐝 𝐜𝐡𝐚𝐧𝐠𝐞
Once nawala na ang ngipin, wala ng mag-“stimulate” ng bone which can cause na magdeteriorate (bone resorption) ang jaw bone or lumiit at maging sanhi para lumubog ang korte ng muka at magmukang matanda.

𝟑. 𝐄𝐚𝐭𝐢𝐧𝐠 𝐢𝐬 𝐦𝐨𝐫𝐞 𝐜𝐨𝐦𝐩𝐥𝐢𝐜𝐚𝐭𝐞𝐝
Kapag nawala ang ngipin sa likuran or ang molars, mas mahihirapan ka ng ngumuya which can cause na ma-limit ang pwede mong kainin.

𝐁𝐚𝐤𝐢𝐭 𝐦𝐚𝐬 𝐦𝐚𝐡𝐚𝐥 𝐚𝐧𝐠 𝐛𝐮𝐧𝐨𝐭 𝐢𝐧 𝐭𝐡𝐞 𝐥𝐨𝐧𝐠 𝐫𝐮𝐧?
1. Kailangan mong palitan ang pustiso mo every 3-5 years. Hindi ito lumuluwag, bagkus, nagkakaroon ka ng “bone resorption” or ang pagimpis ng buto sa panga kaya hindi na sakto ang pustiso

2. Kung gusto mo ng mas permanenteng solution at hindi magccause na umimpis ang bone sa jaw mo, dental implant lamang ang solusyon... which is mas mahal ng di hamak sa pasta or root canal treatment

3. Aside sa cost, hindi ka na makakaenjoy na kainin ang mga usual na gusto mong kaininin kapag naka denture. Mas hindi ka na din confident na ngumiti o magsalita kapag naka pustiso dahil sa takot na ito ay matanggal.

Kaya naman mas nire-recommend pa din ng mga dentista na isave ang ngipin through “pasta” (composite restorations) or Root Canal Therapy hangga’t kaya.

- minsan din after mag braces sinasuggest po namin na magveneers. For some specific reason.

Direct and Indirect.

This is made outside the mouth.
There are different kinds of materials to be used.

EMAX is the best kind to use for such. Highly esthetic and highly durable. And highly expensive. 😅 But IT'S WORTH YOUR MONEY.

If the veneers is done well and maganda pag kakalagay. AT NAAALAGAAN NG MAAYOS. HINDI PO KINAKAGAT SA MATIGAS. It lasts for years and years. At most 20yrs or more.

Then DIRECT.
This one is done inside your mouth. It's done on the spot.

Eto po yung sinasabi nilang papasok kang pangit lalabas kang maganda. ✌️✌️😅😁 Ganun kabilis. 😅😅

COMPOSITE ang material na ginagamit dito. Tulad po ng picture sa post. It's Highly esthetic but it's the least durable. And least costly compared to the rest kinds of veneers. Usually, it last for few months or at most 1yr-3yrs. Kung maalagaan naman.

▶️Impacted tooth are MOSTLY due to lack of space. SOMETIMES, tooth developed in a wrong angle or position.

Do all impacted tooth need to be removed?

▶️No, removal of impacted tooth is necessary if it is causing a problem or required by an institution.

What happens if impacted tooth is not removed?

▶️Depends on the position and how impacted it is. But generally, can cause infection, cyst, and can damage adjacent tooth

Fixed bridge work

Patient had a Maryland for his missing front tooth (not done in our clinic). But it keeps on falling off. Patient decided to stop using it.

He then came to our clinic for other option.

🔺 MARYLand and Rochette bridge is a Good treatment plan for a conservative reduction of adjacent tooth.
❔ Why it failed to this patient?. Not enough reduction was done.

🔺 Removable partial denture is also an option to conserve the adjacent tooth
❔ But not IDEAL for a 1-2 missing teeth. Not unless the patient insisted a removable prosthesis or there are other factors involved.

🔺 Fixed bridge is one of the best option for this case. It provides the best comfort, esthetic, and function.
❔ Not ideal if the patient's hygiene is very poor and bone support is not Good.

🔺 Implant, if the patient has no issue with the price 😁😁
❔ One of the best and ideal treatment. But has alot of factors before a patient becomes a candidate for this treatment.
