Dear All FCIE Locators,

Sorry for this late notice. As per decision of FCIE Management, we are cancelling the vaccination schedule tomorrow, November 16, 2021 and will resume the schedule by Friday, November 19, 2021. The reason is due to the request of one of our locators, I and CKR Garments (formerly Kwanglim) which to date, they still have 400 UNVACCINATED workers.

In order to accommodate their request and considering that we have limited time to prepare on this including the coordination with the LGUs that's why we've come up to reschedule it on Friday. We also just received the confirmation from the Provincial Health Office that they can still provide additional vaccines for this purpose as we all remain on our common goal to vaccinate all the workers inside FCIE.

Again, the vaccination program will be rescheduled on FRIDAY, NOVEMBER 19, 2021.

To all CSE employees,
Kindly be informed that there will be another batch of for vaccine in fcie on Nov 16, 2021, tuesday.
To those who are yet vaccinated, including family & friends, pls register to CSE HR & give the detsils such as full name, birthday, cp no. & brgy. First come, first serve basis.

Vaccination Site: FCIE Sports Center
Our time slot schedule is at 10am but is requested to be in FCIE Sports Complex (Gym) at 9am.

We remind that the COVAX REGISTRATION FORM as distributed by our company nurse/department clerks, must be filled-out already on the 1st portion, 'VACCINEEs INFORMATION.

This shall be submitted to our HR/company nurse for pre-screening of details (1st portion only - vaccinee information).

Pls bring your own ballpen.

Also pls be reminded to follow the following:
1. Wear your face mask & face shield.
2. Always observe social distancing & proper line-up.
3. Prepare yourself for a well-rested & good health body condition:
a. have enough sleep at least 8 hours.
b. drink at least 8 hours of water
c. make sure that your stomach is full. You may bring your water.
d. do not take alcohol drink before & after vaccination.

We expect that you will comply on above mentioned time schedule. And pls. inform your family included in the master list as well.

Do not wait for any text as you are already in the CSE unvaccinated master list. Just pls. bring your company ID or any valid ID.

This refers to the text blast received by all locators on the cancellation of the vaccination schedule supposed to be tomorrow, November 6, 2021.
We are formally informing you all that the said schedule is OFFICIALLY TEMPORARILY CANCELLED.

We are asking your kind and usual help to properly disseminate this information to your workers who are included on the said masterlist to avoid any confusion. For those companies who have settled the payment for OT Fee yesterday, please refund it to our office today c/o the undersigned and please bring the issued Acknowledgement Receipt.

Rest assured that any update on the coming days regarding this vaccination roll-out will be informed to your respective companies ahead of time.
