Incoming first-year students with available Laboratory results may proceed anytime from Monday to Friday 8:00 am -5:00 pm at BPSU DC Health Services Unit (Medical Clinic) for Physical examinations to acquire a compliance slip needed for enrollment. Please bring a 1pc 2x2 picture and 1pc long brown envelope.

>Go to BPSU Affiliated Laboratory / Diagnostic Clinic (BATOL-CANLAS DIAGNOSTIC CENTER) for the laboratory examination (CHEST X-RAY, CBC PC, URINALYSIS, and HBSAG)
>Medical Examination Form can be downloaded at https://bit.ly/3wQqCNx or scan the QR CODE below, or you may obtain a copy from the campus nurse.
>Fill out the Medical Examination Form and attach a 2x2 picture, with laboratory test results. Secure in a long brown envelope.
>Proceed to Medical Screening and Physical Examination at the campus clinic you will enroll in.
>Secure a Compliance Slip from the campus nurse after the Physical Examination.
>Proceed to the Registrar's Office for enrolment.

Pre-Enrollment Medical Guidelines for Incoming First Year and Transferee Students

To all Incoming First-year students and transferees of BPSU Dinalupihan Campus who have confirmed their enrolment with the Registrar's Office must undergo a Medical Examination before enrolment.

Guidelines are posted below:

Mass Blood Donation Drive

We will conduct a Mass Blood Donation Drive at BPSU Dinalupihan Campus, in collaboration with Bataan General Hospital and Medical Center (BGHMC), Dinalupihan Campus CSG, NSTP, and Health Services Unit.

To be held on May 17, 2024, from 9 AM to 3 PM at Isolation Room, Academic Building 1. Employees and students are encouraged to participate in this drive to help and save lives. See you there!
