ANNOUNCEMENT:

TO ALL INCOMING FIRST YEAR STUDENTS OF SORSU MAGALLANES CAMPUS FOR SY 2023-2024

THE SORSOGON STATE UNIVERSITY MAGALLANES CAMPUS will be accepting enrollees for SY 2023-2024 on July 3-28, 2023.
Hence, the Health Services Unit will be requiring documents needed for enrollment.

WHAT ARE THE REQUIREMENTS:
1. Medical Consultation Form (copy of this form is available at the school clinic, Dito n Po mg fill up sa clinic once available na chest x-ray and medical certificate from a government physician)
2. Dental Consultation Form (copy of this form is available at the clinic, no need na po na pumunta sa dentist, Dito n Po mg fill up sa clinic once available na Po Ang chest x-ray and medical certificate issued by a government physician)
3. Medical Certificate issued by a Government Physician
4. Vaccination card (if available)
5. Chest Xray Result/reading (valid within 6 months prior to the scheduled date of Medical examination)
6. Admission Slip
7. Long Brown Envelope with your name (Surname, First Name, Middle name) and course

Please get your booster shot on December 5 2022 at the SorSU Magallanes Campus Auditorium

Mag pa xray po muna bago mgduman sa government physician for your medical certificate..

To all incoming 1st year students wag po kayo mgssubmit ng vaccination card lang dpat po ksma n po yung other requirements na kailangan nyo isubmit. Thank you!

1.xray result
2.medical certificate (dpat po mauna ang xray para dala nyo n ito sa doctor n mg mmedical sainyo)
3.ung ibng forms po(dental consultation record and medical consultation record, available po ito online or sa clinic)

Attention: To all SSU Magallanes Campus students, philhealth registration is no longer required for face to face classes. No need n po mgapply for philhealth registration.
"Upon the recommendation of CHED, Section IV, item H of the CHED DOH joint memorandum circular no 2021-004 pertaining to the medical insurance for students is hereby repealed"
