Hi, Lasallians!

📝 For those seeking to have their medical certificate validated, you may have your approved absences validated once classes resume through our onsite Clinic.

You may kindly inform your respective professors that the delay of the submission of your approved absence is due to the class suspensions.

Hope everyone stays safe and well, kindly message the USG Student Services team if you will be needing any kind of assistance. 💚

📢 Students must conduct in-person transactions for medical certificate validation with the Health Services Office (HSO). 🏥

📌 This is essential for processing excused absences and modified PE class requests. Here’s what you need to know:

📄 Required Documents:
→ Hard copy of the original Medical Certificate or Certificate of Confinement
→ Supporting documents (e.g. prescription, diagnostic work-up result/s)
→ Photocopy of the medical certificate and supporting documents

📝 Validation Process:
→ Submit Documents: Process your request with the HSO within 7 working days from your return to classes.
→ Receive Endorsement: Once validated, HSO will issue an endorsement (yellow) slip.
→ Final Submission: Submit the HSO yellow slip and a photocopy of the validated medical certificate to the office of the Associate Dean.
