1) INTELLICARE- request LOA thru their hotline; pls send pictures of government ID and Intellicare ID.

2) MEDICARD- thru the MACE app or thru their hotline; take note of OTP.

3) MAXICARE-thru the member gateway or thru their hotline

Send the LOA to this page for scheduling. LOA must be secured before we schedule you for teleconsult.

If you have questions on how to request for LOA, kindly direct them to your company’s HR or thru your respective HMO.

ISOLATION is the term used for either COVID swab positive individuals or SYMPTOMATIC individuals.

QUARANTINE is the term used for those who came in contact with COVID positive individuals, but are ASYMPTOMATIC. These are people who MAY develop infection and risk further spread of the virus.

Please take note that the length of the ISOLATION/QUARANTINE period for the general public has not changed.

When to do HOME vs FACILITY ISOLATION.
