CLAIM FORM 4 (CF4) ADVISORY

Please be advised that we are on the trial period for the CF4 requirements of PhilHealth. We are waiting for the approval of Philhealth main office on the CF4 XML output generated by the system. Rest assured that we will keep you posted on the status.

For the transition period given by PhilHealth (March 1 to 31, 2019), you may contact your PRO (PhilHealth Regional Office) to request for a CF4 generator kit. This is an offline application provided by PhilHealth which you can use temporarily to generate an XML format of CF4.

NOTE: PhilHealth requires CF4 to be transmitted in XML format. For the MedSys eClaims users, you will attach the generated XML output when submitting claims. This is applicable to all admissions starting March 1, 2019.
