1. Go to the JSMJC Website: https://jsmjcrizal.com , click the ADMISSION Tab and select your Grade Level

2. Fill out the online Registration Form (make sure you are logged in to your Gmail account)

3. Upload a scanned copy or clear digital photo of your previous grade level Report Card (other documents may be submitted personally).

4. Attach a 2x2 ID Picture in white polo shirt and with white background then click submit.

5. Wait for the email confirmation of your Tuition Fee Assessment and Schedule of Appointment.

6. Proceed to the Admissions Office on the day of your appointment to claim your Assessment Form.

7. Pay your school fees in 2 ways:

(A) ONSITE PAYMENT - Pay at the SHS Cashier. Secure the Official Receipt and present it to claim your P.E. Uniform before the Opening of Classes.

(B) ONLINE PAYMENT - Pay through Online Banking

GENERAL REMINDERS:
1. Make sure to login to your Google/Gmail Account to access the Online Registration Form.
2. If email confirmation is not received 2 days after completing the online registration, check your email spam/junk folder or register again after 2 days. Do not register multiple times on the same day.
3. Only parents/guardians are allowed to transact at the Admissions Office
4. On the day of your appointment, temperature check and hand sanitation shall be done upon entrance to the school.

GRADE 12 - June 9, 2021 (WEDNESDAY)
GRADE 11 - June 10, 2021 (THURSDAY)

‼️IMPORTANT‼️

1. Strictly adhere to your schedule to avoid crowding. The schedule is based on your section and surname. Check timetable and room assignment below.

2. Wear FACEMASK and face shield at all times. Bring your own pen and student copy of your fully-accomplished clearance (hardcopy and/or softcopy)

3. Upon entry, have your body temperature checked by the security personnel.

4. Proceed to the waiting room. Observe social distancing while waiting for your turn.

5. Receive the Report Card from the Class Adviser. Leave the venue once done. Loitering after receiving the report card is prohibited.

6. Grade 11 (incoming Grade 12) students may opt to proceed to the ADMISSIONS OFFICE to PRE-REGISTER for SY 2021-2022. Likewise, Grade 10 (incoming Grade 11) students may also proceed to the Admissions Office for RESERVATION or EARLY ENROLLMENT.
