This ECG tracing belongs to A-43-year old nursing sister in a public hospital. She has diabetes well controlled on Metformin 500 mg OD. She C/O sinking of heart and epigastric discomfort for 1-2 days. Never experienced any palpitation, dyspnea or chest pain previously. She was referred to tertiary care cardiac centre and underwent Coronary Angiography and Echicardiography which were both normal. Routine metabolic profile was all normal. She has been advised further work up and intervention.

1. What's the ECG abnormality?
2. What further intervention or medical treatment is recommended if she has had no symptoms related to this condition.

A 64-year-old male receiving treatment for DM, HTN, IHD. He is on Amiodarone 200 mg daily for recurrent Atrial fibrillation. He presents with H/O palpitation for 1-2 hours. He has no dyspnea or chest pain. BP is 110/70 mm Hg. Trop I is negative.

What's the ECG abnormality?
What immediate treatment is recommended?
What further work up do you recommend?

This 50-year-old lady has progressively increasing painless swelling of left leg for past 6-7 years.
She is normotensive and normoglycemic and has no travel history outside Pakistan.
She is afebrile. Skin over her shin and foot is thick, hard, and dry.
Venous Doppler of leg shows patent venous system and flow.
Few small lymph nodes in inguinal region.

What's the differential Diagnosis?
How should she be worked up?

Disease has a high mortality 98%. Public awareness, importance of chlorination of water supplies was emphasized.
Key to better outcome is early diagnosis and treatment within 5 days of symptom onset.
Wet mount CSF microscopy of freshly obtained sample is key to diagnosis.
Treatment involves multipronged antifungal, antibacterial and antibiotics.
We are sharing the presentation here and @⁨Dr. Arif Nadeem⁩ has permitted to be used by all for presentation in there respective units/ teams to sensitize junior doctors to think of this diagnosis and proceed briskly.
