SCALING AND POLSIHING PROCEDURE:

Preventive dentistry includes a range of actions you can take yourself, such as brushing and flossing twice daily, as well as eating a healthy diet. Cleaning teeth is one of the simplest ways of maintaining oral health.
Brushing and flossing help to remove food debris and sticky deposits caused by sugars and starches that attract acid-producing bacteria. A Physical Examination:
Following a consultation where you can discuss with the dentist any health concerns you have, your mouth will then be examined. We will check for signs of tooth decay, gum problems, or other diseases and we may take x-rays.
Scale and Polish:
The next step is to remove stains and deposits from the surface of the teeth, in between them, and at the gum margins. Large deposits of tartar are removed with an ultrasonic scraper, followed by a manual scaling tool to scrape away any persistent remains.
If you regularly have teeth cleanings with us, it is likely that this process will be reasonably quick. However, if you have a build-up of tartar this will take longer.
Once the deposits of tartar are removed, the teeth are polished and made smooth, which discourages bacteria from adhering to the surface of the teeth. A high-powered toothbrush is used to scrub the teeth with gritty toothpaste, and the teeth are polished to produce a shiny, smooth surface.
The final step is for you to rinse your mouth to remove any debris and grit. We will then protect the teeth with an application of fluoride.
How Often Should I Have Teeth Cleanings?
At a minimum, it is usually recommended that you should have professional teeth cleaning twice a year. However, this very much depends on several issues, such as your oral hygiene regime, whether you have gum or periodontal disease, and your predisposition to oral health problems. When Do I Need a Dental Crown?
A crown is a protective cover that can be made from porcelain, metal, or porcelain-fused-to-metal (PFM). It will protect your tooth against decay, fractures, and wear and tear. A crown is usually needed when a tooth has been damaged by an injury or tooth decay.

The dentist removes tartar from your teeth's surface layer with an ultrasonic scaler or files, then smooths out any rough spots on the tooth roots.
C. APPLICATION OF PASTE:

ROOT CANALS:
Root canal treatment at Dr. Amna's Dental Clinic is a less intrusive option than tooth extraction. Despite its negative reputation, it will not be as big of an imposition to have the infected root canal removed rather than having your tooth extracted and replaced with dentures or implants, which can be costly.
What is a Root canal?
The procedure involves removing all infected tissue from inside a tooth before cleaning out any debris that builds up in there over time. Once this has been completed, your dentist will seal off access to the hollow part of the chamber with either cement or crowns depending on what's best for you as an individual patient.
Although many people are anxious about the thought of root canal treatment, there really is no need. It is similar to a filling in terms of discomfort—but it just takes longer. This is because all the soft tissue must be removed from the center of the tooth, as well as down each root and root canal.
How Do You Know You Need a Root Canal?
A tooth can become infected by bacteria if the soft center (pulp chamber) becomes exposed. This happens when damage to the hard shell of the tooth allows bacteria to reach the pulp chamber.
Most people experience some degree of pain or discomfort when endodontic treatment is necessary (although some experience no symptoms). The main indications that a root canal may be needed include:
A. PAIN
For some people, the pain of a toothache can be mild and non-existent. For others, it may come in waves or is persistent throughout their mouths with no specific site. It's possible for your jawline as well as other teeth to feel sore—which isn't uncommon given that they are all part of the same mouth!
B. TOOTH SENSTIVITY

What Are Braces For?
Braces are used to straighten the teeth to correct a misaligned jaw (malocclusion) and crooked teeth, typically caused by overcrowding. Treatment with braces is most commonly performed during adolescence, but with the interest in esthetics, more adults are having braces fitted later in life. The success rates of braces are good.
How Do Braces Work?
By exerting gentle, constant pressure on the teeth for extended periods of time the braces force the teeth to move. The braces are regularly adjusted, making them a little tighter each time so the teeth continue to move to their correct positions.
Types of Braces
There are several types of braces available.
Braces consist of a metal or ceramic bracket that is affixed to the front of each tooth. A wire is connected to the brackets that are adjusted periodically to put pressure on the teeth, forcing them to move.
Metal braces are the least expensive, but also the most visible. An alternative is with ceramic, clear brackets, which are slightly more expensive but are prone to discoloration.
Invisible braces, called aligners, are virtually undetectable. They are made from clear plastic and are molded to fit over your teeth. However, they are removable and so for anyone likely to forget to wear them, or even lose them, they may not be the best option—especially as they must be worn for 22 hours a day.
Do Braces Hurt?
You won’t feel any pain while they are being fitted—which can take around 2 hours for traditional and lingual braces. However, your teeth may feel “on edge” as a result of the pressure on them, which may take several days to subside.
Every time you have the wires tightened on your braces you are likely to feel this discomfort for a few days. You may have to adjust your diet during this time, eating only soft foods that do not require you to chew.
The orthodontist can give you an indication of how long the treatment will take. On average, it is 18-24 months with braces, followed by a removable retainer, which will need to be worn all day, every day for anything between 4 and 12 months.

Understandably, you may feel apprehensive about having your natural teeth removed. The actual procedure itself may be worrying enough.
What Happens During a Tooth Extraction Procedure?
A tooth extraction is advised if dental treatments such as fillings, crowns, or a root canal cannot save or restore a tooth. Tooth extractions are common procedures that are either a simple extraction or a surgical extraction.
Simple Extractions
Most tooth removal procedures are carried out as simple extractions. This means that no incisions are made in the gum. During this procedure, a local anesthetic is applied to numb the area around the tooth. It is loosened by rocking it back and forth and once loose enough the tooth is removed easily using forceps.
Surgical Extractions
A surgical extraction is required for larger teeth, such as the wisdom teeth. A surgical procedure is also needed for impacted teeth (those that have not erupted through the gum line), or if a tooth has broken below the gum line.
An incision is required in the gum line during a surgical extraction, which will require stitching. Before the decision is made to perform a surgical extraction, x-rays will be carried out to confirm that surgery is the best court of action. Sometimes, a tooth may need to be broken into pieces before it can be removed.
Surgical extractions are performed via a local anesthetic or conscious sedation. This is to make sure that a patient does not feel anything during the procedure.
What Should I Expect During the Recovery Period?
We will give you specific guidelines to aid your recovery from your tooth extraction procedure. It is important to adhere to these to ensure you have a quick recovery while avoiding any complications.
Immediately after your extraction, the tooth socket is likely to be bleeding. We will place gauze over the area, and you will be required to bite down gently on it for up to an hour to stem the bleeding and help to form a blood clot.
You should avoid rinsing your mouth or eating and drinking for several hours afterward so as not to dislodge any clots, and drink only lukewarm liquids and eat soft foods until the bleeding has stopped. Avoid smoking and drinking alcohol as these inhibit the healing process. Your face may be bruised and swollen following your extraction—particularly after a surgical extraction—but applying ice packs to the area can help to minimize swelling and alleviate any discomfort.

Introducing the case: Cutaneous Leishmaniasis and Oral Manifestations.
This patient presented to the OPD with severe cutaneous fungating and ulcerating lesions, to be evaluated for oral sores. A report from Agha Khan Hospital Karachi (2019) showed that a lesion similar to the facial one had been present on the hand for the past 8 years (2011). Biopsy was taken, and sections revealed hyperkeratotic skin with underlying tissue, with focally ulcerated epidermis and fibrino-purulent exudate. Histiocytes and occasional multinucleated giant cells could be appreciated, with the granulomas surrounded by lymphoplasmacytic infiltrate. Special stains “Ziehl Neelsen” and “PASD” were negative for AFB and fungal organisms.
The diagnosis established was Chronic granulomatous inflammation with necrosis, with Tuberculosis in differential.
The case went uncharted for almost an entire year, then in August 2020, the patient showed into the OPD again, this time in BBH. Reason was the surprising spread of the lesions to the face of the patient, with the complaint of painful swelling of face involving lips from 3 months. PT had a history of multiple/various admissions at derma wards of Lhr and Islamabad.
Biopsies of both the face and the hand were taken. Face biopsy results revealed CHRONIC GRANULOMATOUS INFLAMMATION, whereas hand biopsy showed HYPERKERATOSIS WITH MILD INFLAMMATORY INFILTRATE. Differential diagnosis established after clinical correlation was cutaneous TB and cutaneous leishmaniasis. Pt was advised IGRA but he could not afford these.
Within the same month, the patient got Leishmania Smear (LT Bodies) test conducted to confirm whether Leishmania Tropica was present or not. The test came out negative (i.e. LT bodies were not seen).
Further referral to AFIP Rwp was advised (Sep 2020), where biopsy was taken again to evaluate for fungi (Periodic Acid Schiff was negative), acid-fast bacilli (Ziehl-Neelsen stain was also negative), and immunochemistry was done for CD1a (also negative for Leishman-Donovan bodies). Mantoux Test was conducted, which came out negative as well.
Patient returned to Kashmir, Mirpur and got admitted in Mohiuddin Teaching Hospital, with the differential of Lupus Vulgaris and cutaneous Leishmania. IGRA was conducted and came out negative. A punch biopsy with PAS was performed which indicated chronic granulomatous inflammation.
The patient then presented to a private clinician, where he was treated and two months down the line, his swelling of face and hands had decreased, but he had developed crusted lesions on face with ulceration in buccal mucosa. Comorbidities like HTN and diabetes made his healing and recovery slow. He was referred to medical specialist for the treatment of diabetes but unfortunately, the patient refused due to financial issues. By the time the patient presented to our clinic for eval of the oral symptoms, the lesion had extended to right brow as well, which was sent for H/p.
Oral manifestations were limited mouth opening, with no ulcers on the retro molar areas. We recommended palliative medication for oral symptoms but the major treatment regime is to be managed by medical and derma doctors. The treatment is under process and we hope the patient recovers soon!

THE HISTORY OF DENTISTRY:
In the world we live, there is a general unpleasantness associated with dentistry. One of the reasons can be the barbaric surgeons who lived and worked in the 1600’s and 1700’s. They used to extract teeth without anesthesia and it is said that poor populace used to sell it’s teeth and the rich got them implanted in their empty sockets. Some say that drums were clanged to engulf patient's screams. Evidently enough, for generations, dentistry has had a nuance of unpleasantness, so perhaps, the stigma still prevails at large and many people still fear to visit a dentist.
Let us plunge ourselves a little into the history of dentistry.
The very first evidence of dentistry as a practice comes from the Indus Valley Civilization. From 5000 B.C., a Sumerian text describes tooth worms as a cause of dental decay, an idea that was proven false in the 1700s.
Back to the ancient Greece, Hippocrates and Aristotle wrote about dentistry, specifically about treating decaying teeth. The first book ever published on dentistry was in 1530, named “The Little Medicinal Book for All Kinds of Diseases and Infirmities of the Teeth”.
By the 1700s, dentistry had become a more defined profession. In the year 1723, Pierre Fauchard, a French surgeon who is credited as the “Father of Modern Dentistry”, introduced the idea of dental fillings and the use of dental prosthesis. It was him who identified that acids from sugar led to tooth decay. This was a big achievement in its time.
Forward to 1840, when the first ever dental college, called the Baltimore College of Dental Surgery, was inaugurated, leading towards more insight on dentistry.
By 1873, Colgate had mass produced the first toothpaste, and production of toothbrushes followed a few years later.
There is somewhat a surprising thing, though. Most of the Americans did not adopt good brushing habits until after World War II. This too, happened only when soldiers stationed abroad brought back the concept of good oral health to their homeland.
Nitrous oxide was used as an anesthetic agent in 1844 by Horace Wells for the first time. Horace was a dental surgeon in Hartford, Connecticut, USA. With the origin and development of local anaesthesia, pain experience lessened to a greater extent if not gone altogether. Today, multiple other techniques are implemented to relax a patient before any kind of dental procedure. Any treatment that could cause pain can not be imagined without anaesthesia. Yet here we are, with many of us still dreading a visit to a dentist.
