Beware of peanuts and other loose foreign objects around the house when you have small children!

Yes this was once someone’s lungs. The lungs of a 5 year old child. And guess the object lodged in the airway(trachea)… A peanut!!!! The reason why I advise parents not to have children under 5 eat peanuts in fact it should not be in the house at all if you have small children. Too many times I’ve seen children requiring thoracotomies (surgical opening of the chest cavity) to remove peanuts lodges in the lower airways in the lungs. The shape and smoothness of the peanut and the oils it contains make it very slippery and easy to slip down the wrong pipe going down the windpipe instead of the food pipe. Unfortunately the lungs pictured belong to a child that died from asphyxiation from said peanut!

Cold vs Allergies(Nasal allergy symptoms) - With the world “seemingly” on the back end of the pandemic and the Sahara dust “seemingly” a mainstay in our atmosphere. Here’s a common dilemma amongst my patients. “Doc!? How do I know if I have a cold or allergies?” Here’s a few things you should consider:
1. Sometimes it’s very hard to tell because the symptoms are so similar and some of the treatment is as well
2. Always be very careful cause a lot of Covid cases started with the patient having “allergies”
3. When in doubt see your doctor/ get tested for Covid 19
Some of the treatments we use may include:
- nasal steroids- the mainstay of long term allergy treatment
- Antihistamine(oral) help with some of the classic allergy symptoms (itchy eyes, ears, throat; runny nose)
- Topical nasal antihistamine(not easily available in Trinidad and Tobago)
- Oral steroids(prednisolone)- the strongest medication we can give to relieve allergies
- Avoidance therapy- staying away from allergens like dust(impossible with the Sahara dust), animal dander, pollen, mold using hypoallergenic pillows, air purifiers etc
- Diet- abstaining from dairy products(milk and cheese and yes ice cream too😭😩)
- Oral decongestant(phenylephrine)- usually a constituent of most over the counter cold and sinus medication
- Topical decongestant(otrivin/afrin)- provides great and instantaneous relief however should not be used for more than 5 days straight or else you can get rhinitis medicamentosa or rebound nasal blockage worse and more prolonged than initially

#420 In keeping with the recent 420 proceedings and given how popular the alternative medicine craze is currently I will try to shed light on a “taboo” but popular topic. How helpful is ma*****na in treatment of diseases particularly ENT and respiratory conditions? We must remember that:

SMOKING IN ANY FORM IS IRRITATING TO THE AIRWAY AND UPPER AERODIGESTIVE TRACT AND CAN WORSEN RESPIRATORY DISEASES AND IS A MAJOR RISK FACTOR FOR HEAD AND NECK AND LUNG CANCER!

Now that we’ve gotten that out of the way. We have long heard about the anti inflammatory and chronic pain relieving qualities of cannabis particularly the CBD(Cannabidiol) component. With the recent worldwide trend to decriminalize ma*****na use the following slides present a bit of information on its usefulness and harmfulness.
Remember not all ma*****na contains the same composition of compounds and some strains may cause more psychosis than anxiolytic effects; And different preparations may cause different reactions
