Texas Nurse Care Home Health is looking for an experienced RN and QA LVN full time.

Duties of the RN include but are not limited to; individualized care for our residents/patients, along with medication management, medication administration, observing and monitoring residents/patients' conditions, maintaining accurate records and making physician recommendations. Other duties may include; SOC Start of Care, Evaluation, Coordination of care with community health providers, On site injections as applicable, Diabetes management and Educating residents/ patients on medication and disease management. **VACCINATION REQUIRED**

QA LVN experience in LVN notes and OASIS, Telephone Orders, Coordination of Care, and Communication Notes.
