Quality Healthcare Development (QHD) is looking for full-time Quality Improvement Coordinator to provide support and project coordination for the quality improvement functional area. The Quality Improvement Coordinator works closely with the other members of the Quality team to improve patient outcomes and organization contract performance. This role involves performing HIPAA-compliant medical record reviews and collecting documentation from electronic medical records to ensure patients are receiving preventive care screenings.

Reports to: Quality Director

Location: North Charleston, SC

Essential Functions:

Provides support for the quality improvement functional area.
Follow up with providers and staff on medication adherence for patients.
Assist with quality measure gap closure.
Completes special projects or assignments as needed to meet initiatives and/or objectives of department.
Assists and coordinates Quality Improvement projects as needed.
Maintains all department files in an organized, accurate manner.
Maintains and secures files of Quality Improvement activities.
Directs communication to the appropriate staff.
Coordinates provider profiling activities.
May be required to travel to provider offices to collect charts in support of HEDIS and other chart collection initiatives
Provider any additional outreach and/or support to patients as needed
Additional duties as assigned
Candidate Education:

Required A High School or GED
Required or equivalent work experience
Candidate Experience:

Required 2 years of experience in medical office, HMO or hospital environment performing administrative duties with a clinical focus
Required Other Experience with medical terminology
Advanced Demonstrated written communication skills
Advanced Demonstrated interpersonal/verbal communication skills
Intermediate Ability to communicate and make recommendations to upper management
Intermediate Ability to multi-task and maintain multiple projects simultaneously
Intermediate Ability to work in a fast-paced environment with changing priorities
Intermediate Other Ability to communicate effectively with people at all levels
Intermediate Other Ability to arrange their own workday and work independently
Ability to strategize and integrate ideas for different practice settings
Technical Skills:

Proficient Experience in Microsoft Word, Excel, PowerPoint
Electronic Medical Record Experience
