UIC has partnered with AgeOptions to conduct a survey investigating the needs of adults (age 50+) and caregivers of older adults in suburban Cook County.

The H.O.P.E. by AgeOptions team had continued success in the month of January 2022. The team transitioned an additional 4 Members from Skilled Nursing Facilities into the least restrictive residences, within the Communities of their choice. Year-to-date they have transitioned 31 Members, meeting their year-to-date FY22 goal.
