Purpose: To empower, support, and assist developmentally disabled individuals living in the community and to enable those individuals to live and participate in their communities as fully as possible.

Work Hours: Full time or Part time, scheduled by Residential Coordinator based on needs of assigned work sites.

Environment: Primary - Individual residences might include exposer to cigarette smoke and common household pets.
Secondary - community/social events, community services, retail establishments, medical offices/facilities.

Physical Demands: Physical activity related to normal household chores, physical assistance with mobility and transfers may be required according to specific individual's needs (lifting, crouching, bending, pushing, and periods of standing, sitting, or driving in excess of 30 minutes.)

Equipment: Operation of appliances related to normal household environments. Adaptive equipment as required, including but no limited to: wheelchairs, electric lifts, safety/protective gear, physical braces/supports and communication devices.

Requirements:
+ High School diploma or GED
+ Valid Ohio Driver's License
+ Dependable transportation with current automobile liability insurance
+ Current CPR and First Aid Certifications
+ Pass criminal background check
+Successful completion of employer provided training
+Ability to effectively communicate with supervisors, colleagues and the person(s) receiving services; be able to speak, read, write and understand the English language.

Purpose: Manage and develop the day-to-day operations of the assigned site and individualized services for the adults with developmental disabilities residing therein.

Work Hours: Full-time, variable schedule. Hours are based on the needs and circumstances of the individual worksites. The schedule may include weekends, nights, holidays, and periods of activity in excess of eight (8) consecutive hours.

Environment: (1) Individual residences, rural and urban; may include exposure to cigarette smoke and common household pets; (2) Standard office settings; (3) Community/social events both private and public, community services, retail establishments, medical offices/facilities; (4) Travel beyond the residential city/state of the individual.

Physical Demands: Physical activity related to normal household chores; Physical assistance with mobility and transfers according to specific individual's needs to include: lifting, crouching, bending, pushing, and periods of standing, sitting, or driving in excess of thirty (3) minutes; Exposure to inclement weather conditions.

Equipment: Standard office equipment include but is not limited to copy machine, fax, cell phone, and desktop computer.

Requirements:

o High School diploma or GED

o Maintain valid Ohio Driver's license with less than six (6) violation points

o Maintain dependable transportation with current automobile liability insurance

o Maintain current CPR and First Aid certification

o Submit to and pass a criminal background check (to be completed once every three [3] years)

o Successful completion of employer-provided training

o Minimum two (2) years experience working with DD or Mental Health clientele

Skills andAptitudes:

Basic computer skills to include word processing, e-mail operations, and internet familiarity
Proficient reading, writing, and speaking skills in English
Perform routine math functions
Respond supportively and empathetically to individuals with a variety of developmental disabilities from a variety of cultural and socioeconomic backgrounds
Appropriately and effectively assist individuals during crisis situations
Effectively present information and respond to questions from support staff, managers, clients, medical professionals, family/guardians, law enforcement, and other professionals and representatives as needed
Assess individual, group, and situational needs in dynamic settings and facilitate the development of systems to address needs and changes
Exercise independent judgment and take action/s to ensure health and safety
Display expert time management skills
EssentialJob Responsibilities:

o Schedule and coordinate program activities per Individual's Support Plan {ISP) and Behavior Support Plan (BSP)

o Provide training and supervision of Direct Support Staff at the assigned site; Empower and engage employees in carrying out the ECHO mission, team goals, team roles, and team practices

o Perform performance evaluations and corrective actions as required and ensure Direct Support Staff are current with all site-specific training

o Coordinate all client medical appointments and attend as needed to ensure continuity of care

o Coordinate medication administration with the Nursing Service and ensure all records are accurate and current; Oversee food and supply acquisitions for sites; ensure special dietary needs are followed per ISP

o Provide support and documentation as requested by ECHO financial Coordinator for financial activity including house budget, home petty cash, and in-home client personal funds. Assist with the preparation of annual taxes for persons served, food stamp applications, redeterminations, etc. as needed; Comply with all financial mandates.

o Complete and monitor for accurate timekeeping records for the Direct Support Staff; Ensure staff ratios are in compliance; Provide on-call support as scheduled/needed.

o Ensure activities, operations and documentation are carried out in compliance with ECHO policies and procedures, regulatory and accreditation organizations

o Facilitate the development and implementation of team goals through meeting attendance and ongoing communication

o Maintain positive business relationships with members of the client's support team

o Complete ancillary functions as needed or directed by the Director and/or ECHO's Executive Director or CEO.
