I hope everyone had a great Father’s Day. I did not know I was going to be brought to tears with this letter from my daughter, but I thought I would share it. I have a 13-year-old teenager and sometimes it’s very challenging to deal with her and you wonder sometimes if you’re doing the best you can.It can be very chaotic being a father to a teenager. Sometimes you get a little confirmation you’re pushing them in the right direction even though they won’t admit it most of the time. I hope everyone’s Father’s Day was as good as mine yesterday. if not reach out to your kid. Life is short. Fix the situation . Say you’re sorry even if you don’t need to.

As screens (televisions, computers, and smartphones/tablets) become an increasingly important part of daily life, many people gradually take on a more slumped posture, which can place added strain on the neck and shoulders, raising the risk for neck pain and headaches. Luckily, it’s possible to improve forward head posture, rounded shoulder posture, and scapular instability with neck-specific exercises and chiropractic care.

In a 2018 study, patients with forward head posture performed either scapular stabilization or neck stabilization exercises for 30 minutes three times a week for four weeks. Participants in both groups experienced improvements related to their craniocervical angle and muscle activity around the upper back and neck, with greater results reported by the scapular stabilization group.

Several studies have shown similar results for improving forward head posture using both scapular and neck stabilization exercises. In another study, high schoolers with forward head posture performed scapular and neck stabilization exercises and exhibited good posture up to four months later.

A 2019 study looked at the effect of a six-week intervention featuring manual therapy and/or stabilizing exercises on 60 women with neck pain and forward head posture. Participants in both the manual therapy/stabilization exercise-combo group and the stabilization exercises-only group reported better outcomes with respect to head posture, pain reduction, and improved function, but the results were best in the combined treatment group. The authors concluded that manual therapy adds a meaningful role to a structured exercise program that addresses scapular and neck instability and forward head and rounded shoulder posture.

The temporomandibular joint (TMJ) is one of the most intricate and frequently utilized joints in the human body, working in conjunction with the internal and external pterygoids, masseter, and temporalis muscles to open and close the jaw, as well as stabilizing the hyoid bone during swallowing and protecting the airway while chewing and talking. Musculoskeletal issues that affect normal TMJ function are characterized under the general term of temporomandibular disorders (TMD). Symptoms associated with TMD include joint noise (clicks, catching, or grinding), pain, altered jaw motion, and restricted opening. While there are several potential causes for TMD linked to the jaw area itself, in recent decades, there has been a growing body of research supporting cervical dysfunction, poor neck posture in particular, as an underlying cause worth investigating in the event standard treatment fails to yield a satisfactory result.

The head is balanced on the neck in alignment with the trunk, as seen from the side. However, because of the increased use of screens and electronic devices, it’s become common for people to exhibit more of a forward head posture. That is, the head rests forward of the trunk. This forces the soft tissues at the rear of the head, neck, and upper back/shoulders to work harder to keep the head upright, elevating the risk for injury to the associated tendons, muscles, ligaments, joints, and nerves—some of which may also play a role in temporomandibular function.

In cases in which a TMD patient presents with either poor neck posture or neck pain, treatment to address the cervical region may help alleviate their jaw pain and other TMD-associated symptoms. Additionally, studies have found that TMD patients with jaw pain lasting longer than three months, increased sensitivity in the masticatory muscles, co-occurring headaches, co-occurring hearing complaints, and more severe and disabling TMD symptoms may also benefit from treatment restoring normal posture and function to the cervical spine. While disk herniations in the lumbar spine are fairly common and may resolve on their own or persist without symptoms, if a herniation places pressure on the spinal cord or nerves exiting the spine, the patient may experience radiating pain into one or both legs. While chiropractic care can help individuals with a herniated lumbar disk, patients often ask what they can do and what they should avoid between visits to facilitate the healing process.

Recommendations can depend on the location of the herniation, but lumbar disk herniation patients should try and remain active within pain tolerances. However, they should avoid activities that involve twisting, such as golf, tennis, or pickle ball. Additionally, they should avoid prolonged bed rest as research has shown such behavior can decondition the muscles in the lower back, which can lead to persistent, chronic pain and disability.

Since MANY activities of daily living require bending (like getting dressed or putting on shoes), a very important modification is to bend the knees and arch the back (poke out the buttocks) prior to bending forward. This is called hip hinging and reduces lumbar disk compression. Interestingly, our disks are like sponges and absorb water during the night. Because of this, an interesting study reported avoiding forward bending in the morning resulted in a faster recovery compared to a group that was not educated on this important point. That means, lie on your back in bed to get dressed in the morning (shoes, socks, pants, etc.).

Neurodynamic stretching, or nerve flossing, can also benefit disk patients. If compression occurs at the L1 or L2 levels, extend the leg back with the knee fully flexed (called a femoral stretch sign). For herniation at L2 or L3, stand or lay sideways, chin up, grasping the ankle and pull the thigh backward to reproduce the radiating symptoms in the front of the thigh. (continued in comments)

For patients considering spine surgery, a recent study that looked at outcomes of 1,088 patients who received a second opinion found that only 34.7% underwent a surgical procedure in the next two years, while 60.8% were recommended to pursue a conservative treatment approach (like chiropractic care). When it comes to a condition like low back pain, does it matter what type of healthcare provider a patient initially visits for treatment? A study published in 2015 looked at this question and found that the type of healthcare provider a patient initially consulted had a dramatic effect on both their short-term and long-term prognosis.

In the study, researchers monitored 719 patients with low back pain, 403 of whom initially consulted with a medical physician and the rest first sought care from a doctor of chiropractic. The results showed that the patients in the chiropractic care group not only experienced a greater reduction in their low back pain, but they were also more satisfied with their experience. Additionally, the average cost of treatment for the chiropractic patients was $368 (US dollars) lower than the average cost of treatment in the group that received care from their medical doctor. The study clearly favored chiropractic care as the initial treatment choice for patients with low back pain.

In a 2019 study, researchers reviewed medical records from over 216,000 patients without a history of opioid use and who had new-onset back pain to see if initial provider choice influenced future prescription narcotic use. The data showed that 22% of patients received a prescription for an opioid in the short-term; however, patients who consulted with a doctor of chiropractic first were even less likely to require either a short-term or long-term prescription than those who visited a medical doctor initially. The authors of the study concluded, “Incentivizing use of conservative therapists may be a strategy to reduce risks of early and long-term opioid use.” (Continued in Comments)

Manipulative Therapy and Low Back Pain.

Among a group of 80 adults with chronic low back pain, researchers observed that those treated with manipulative therapy—a form of hands-on treatment provided by doctors of chiropractic—three times a week for four to six weeks reported significant improvements with respect to pain, anxiety, and sleep quality.
