We would like to thank you for your many years of support. Please know that we are as disappointed as you are in regards to this. We have enjoyed serving our many patients over the years by sharing in your challenges, your successes, and the stories of your journey. This has been a source of the greatest joy for us!

Your medical records will continue to be available to you and you may request them at any time by emailing your request to [email protected]. Please include Patients full Legal Name, Date of Birth, Social Security Number along with a current mailing address on where to send these records. These requests will be fulfilled within 15 business days following receipt.
