"Creating Diverse Partnerships to Promote Oral Medical Care Coordination in Clinical Settings" Thursday, Aug 22 from 3:00 to 4:00 p.m Eastern Time (ET)
The link between oral health and the effective management of chronic conditions such as diabetes and cardiovascular disease is well documented in the literature. This webinar will highlight success stories from five states (CO, CT, ND, SC, and VA) involved in the NACDD project “Partner Actions to Improve Oral Health Outcomes.” These stories demonstrate that state health departments have a role to play in increasing awareness about the impact that poor oral health has on chronic disease and that being successful requires the development of new partnerships and care coordination between oral health and chronic disease programs, primary care providers, and their corresponding community partners. Each of the states took different approaches to achieve similar goals.

Did you know that 63% of Americans have never heard of lupus or know little about its symptoms? NACDD and the Lupus Foundation of America (LFA) are teaming up to change that. By supporting research, education, advocacy, and resources, we're making a real impact on improving lives.
Lupus awareness is crucial year-round, and community health partners play a vital role in increasing visibility and understanding. The LFA is thrilled to announce the launch of their Community Health Worker (CHW) Resource Hub. The Resource Hub was created to broaden lupus awareness outreach to non-clinical professionals like patient navigators and social workers.
