Seeking to provide person-centered, individualized case management, care coordination, advocacy, and I am not set up to meet with individuals in my home.

I will come to your home, meet remotely, or meet with you at an agreed upon community location. I also do not transport individuals in my car, although I am willing to provide bus training and accompany folks to appointments via RideSource and other transportation services. I can help you find/apply for transportation resources if needed.

Hello and Welcome! I am a Certified Qualified Mental Health Professional (QMHP-C) with a Master's degree in Community and Social Psychology and an undergraduate degree in psychology. For more than 25 years I worked in the public mental health field providing counseling, case management, community integration support, and skills training. Working with individuals who were diagnosed with mental health disorders was very rewarding but I also got to see first-hand how difficult it is for individuals and their families to navigate a confusing system with limited resources, and, despite the hard work of the mental health staff, services were often not enough to sustain individuals on a long-term basis in their communities due to an overburdened system. Thus, the idea for providing private, individualized, holistic case management, advocacy, and community integration services arose. I remain passionate about serving individuals with disabilities and would love to advocate for you (or your loved one) and help you (or your loved one) connect with resources that will nourish the body, mind, and spirit. I would also like to help coordinate your care between the resources in your life (e.g., family, legal, medical, mental health) to reduce your (or your loved one’s) burden and ensure that everyone is working together to sustain your (or your loved one’s) wellbeing. Here are some of the many things that I am able to assist with:

• Support with Transitions (e.g. into a new housing situations, or in and out of the hospital)
• Support Applying for Benefits or Accessing Resources
• Support Researching Available Resources for the Best Fit
• Support with Advocating for Services to be Covered by Insurance
• 1:1 Community Coaching to Improve Independence in Accessing Community Resources
• Specializing in Behavioral/Mental Health Disorders (although open to other populations)
• Support Coordinating Services Between All Providers and Supports
• Help with Goal and Care Planning
• Open To Exploring both Alternative Care and Conventional Care Options
