For this lovely 80-year-old, sudden weight loss had wreaked havoc on her face. She wanted a nonsurgical solution with no down time.

Temple filler is one of my favorite procedures - and one for which patients often consult me after seeing the amazing results on their friends!

I love how it transforms the face subtly yet dramatically.

🌟 For this lovely 58-year-old, just one-fifth of a teaspoon of temple filler on each side is the secret to a radiant upper face glow-up.

🌟 During aging, the face appears to rotate downwards. When done right, temple filler corrects this downward pull and restores a smooth transition from forehead to cheek.

🔦 SEASON Spotlight: AI in Aesthetics - What's our Future?

I've been predicting for years now that AI aka Artificial Intelligence may be most successful during patient consultations - to help determine who's the right candidate for a particular procedure or approach.

While that’s a bit less dramatic than the widespread notion that AI could replace us in actually planning and performing treatments, it's perhaps more realistic in the foreseeable future.

This article from the latest edition of the Aesthetic Surgery Journal looks at how well CHAT GPT-4 - latest upgrade of the best-known AI tool - generates responses to questions of varying complexity that are typically posed during cosmetic consultations.

Findings from the authors - Drs. Yi-Xin Sun, Zi-Ming Li, Jiu-Zuo Huang, Nan-ze Yu and Xiao Long:

👉 AI was equally good at generating appropriate responses to nonsurgical and surgical questions.

👉 Not surprisingly, the AI responses were significantly better for questions of low or medium complexity than high.

👉 The biggest shortcomings of the AI consultant were in being completely up to date... and in showing empathy!

Key Take-Homes:

🔑 As always, AI is only as good as the human intelligence that fuels it!

🔑 Quality performance by AI in cosmetic consultations requires regular updates from medical journals and other data sources, training in image recognition, and ongoing performance review by human experts.

🔑 As for empathy, well that may prove a little more challenging!

Mirror, Mirror, Drive Us *Off* the Wall?

Our lives are crowded with mirrors... and not just those that hang on our walls. The modern mirrors of the media - magazine covers, movies, models - bombard us on a daily basis. All reinforce a rigid beauty "norm", an ironic term considering how few of us are normal in this respect!

This can lace the simple act of looking in the mirror with a cocktail of complex emotions - anticipation and apprehension, the hope of fantasy, and the contrast of reality.

Our conflict with mirrors is powerfully expressed in the story of Snow White. The magical talking mirror brings immense pleasure over the years to Snow White's stepmother queen, until the day when it tells her that Snow White's beauty has surpassed her own.

This cartoon is a funny take on that fairytale , but it also holds an important message. The most distressing part of aging is when our faces no longer project the true image of how we feel inside. There's a mismatch between our outer self and inner persona.

As one of my patients told me: "I used to think bad hair days were terrible enough, but now I have bad face days!"

As children, we enjoy a joyful and uncomplicated relationship with mirrors. Think of the pure pleasure of reveling in our own image as we play dress-up.

How can we reclaim this in adult life?

I think it starts with allowing the sweet music of how we see ourselves to rise above how others perceive us. We empower ourselves when we make peace with those myriad mirrors and don’t allow them to judge us.

This is the key to developing our own unique beauty to its fullest, unimpeded and unthreatened by airbrushed or photoshopped fantasies - or by anyone who would hold us to impossible standards.

SEASON Spotlight: Filler Nodules - Why, Where and How?

This Commentary has aroused a lot of interest - thanks to all for the positive feedback!

As promised, here it is, together with the great publication it accompanies from Drs. .velthuis.33 and Cotofana.

👉 Filler lumps and bumps can appear days, weeks, months or even years after injection. As we point out in this Commentary, strategies for avoiding or dealing with them rely currently on "the 3 C's" - case reports, cadaver studies and consensus opinions - which cannot be reliably extrapolated to real-world practice.

👉 Our bridge to true understanding is . Schelke et al. found that all 27 patients they imaged had filler trapped within the fascia - the web of connective tissue surrounding and supporting all our essential structures.

❓How did it get there? Either through Filler Misplacement or Filler Displacement - two new terms we've introduced to replace “Filler Migration", which could imply random occurrence due to products rather than injectors.

👉 Due to passages and connections, filler that's mistakenly injected into the fascia or ends up there due to facial muscle action can shift anywhere from the scalp to the neck. This is how cheek filler can end up in the temples or around the eyes, or chin filler in the neck.

Our Key Take-Homes:

🔑 Injector Responsibility: Filler nodules are under injectors’ control - through selection of the right products for the right purpose, injection into the right tissue layer, and avoidance of over-injection.

🔑 Qualifications Matter: Choose your injector wisely! For every two patients I treat these days, I'll see one with complications from treatment elsewhere. It's the same for every expert dermatologist and plastic surgeon with whom I teach and do research.

🔑 Ultrasound is Our Future: For anyone intimidated by using ultrasound during injections, we've suggested the practical solution of quick scans before and after injection to learn how to place fillers exactly where intended.
