Case of 2 brothers with multiple premolars with Dens Evaginatus.
Prophylactic resin seals were placed for all other premolars. The ones treated with endodontic treatment required placement of a bioceramic material - BC putty due to the extensive apical size secondary to maturation status and resorption associated with periapical pathology.

Mb2’s Day kind of Tuesday

Radix Entomolaris refers to the presence of an additional root, specifically the mandibular molars. While mandibular molars typically have two roots (mesial and distal), Radix Entomolaris manifests as an extra root located distolingually.

Case challenges:

• Anxious patient with a Hyperactive Gag Reflex

• Extreme curvature on the DL root.

• Anatomy

Solutions:

• Oral conscious sedation + Acupressure Adhesive Magnetic Patch for the gagging (Thank you Yehuda Isseroff, DDS for the tip) placed on the labial mental groove.

• Glide path for the curvature.

• Enhanced irrigation -Gentlewave

 

Caries directed - Heat-treated files come to the rescue with guidance to the or***ce facilitated by “ Cold treatment” of the rotary file with a gauze soaked in Alcohol.
I will let the smarter peeps comment on the phase transformation of the metal between the austenite to the martensite phases.

On another day in Endo paradise, 65 yo fine gentleman, had a gold nugget on his tooth which had secondary leakage, buried under this gold nugget were these squids....worth their weight in gold!

The 11th meeting of the International academy of Endodontics was a hit!

I was fortunate to share my passion for managing dental trauma and stretching it’s limits with this lovely group.
