Hi y’all-- wanted to share these AMAZING before & after pics of my sweet patient with melasma and sun spots. HUGE improvement with my favorite prescription-strength compounded lightening cream + sunscreen + strict sun protection. No lasers, no chemical peels (although those can be very beneficial).

**I know the lighting and angles are a little off. We don't have a fancy camera set up in our office—these photos are taken from our clinic iPads— but you can definitely appreciate the improvement! 🤩

P A T C H * T E S T I N G⠀

▫️What is it?▫️⠀
🔹A very useful test to apply when we suspect that a rash is caused by something coming into contact with your skin (an allergic contact dermatitis, or ACD).⠀
🔹Getting a rash to poison ivy or nickel are examples of common ACDs.⠀

▫️Does it hurt?▫️⠀
🔹Not at all! This is NOT the same as allergy prick testing (which tests for reactions to pollen, mold, dust mites, etc), typically performed by allergists. ⠀
🔹In PATCH testing, which is commonly used in dermatology, small STICKERS are placed on your back to see which allergens you may have reactions to (see next photos for a visual). ⠀
🔹In my office, we use the NAC-80 Series which tests you for EIGHTY of the most common allergens. ⠀
🔹Common allergens include nickel, cobalt, fragrance, preservatives (commonly found in cosmetics, personal products such as shampoo, or even clothing). ⠀

▫️What to expect?▫️⠀
🔹Patches get placed on your back on Monday. ⠀
🔹On Wednesday, you return to clinic and we remove the patches and do an initial “read” to check for any reactions (redness/inflammation corresponding to the allergen).⠀
🔹On Friday, you return for the FINAL read. We go through your positive reactions, give you information on each of them, and advise you on what allergens to check for and AVOID. Feel free to bring the products you use at home so we can check!⠀
*You cannot get your back or the patches wet during this 5 day process* This is often the hardest part!⠀

▫️Is it covered by insurance?⠀
🔹Usually, yes!⠀

🔹Not everyone with a rash has an allergic contact dermatitis and patch testing does not always reveal positives. With a thorough history and physical, a board-certified dermatologist will let you know if we think patch testing would be useful!

A good Vitamin C serum should hold its place in your anti-aging skin care routine.⠀

🍋Vitamin C is a potent anti-oxidant—helps with aging skin and dark spots (brightens complexion)⠀
🍋Also helps stimulate collagen in the skin (👋hello youthful looking skin)⠀

✴️Make sure your Vitamin C is kept in a dark container as it gets oxidized and will lose its effect if exposed to light⠀
✴️Concentration: should be at least 10-30% Vitamin C⠀

❇️By far, the most popular serum is CE Ferrulic by . It has added Vitamin E and ferulic acid to help Vitamin C work even better. It’s a splurge item but worth it. Formulation and quality of Vitamin C is super important.⠀

❇️If you have more sensitive skin that is prone to acne or if you have more oily skin, I really like Phloretin CF by . (This is what I personally use.)⠀

🔷Apply 3-4 drops to your face & neck every morning after washing your face (then apply moisturizer/sunscreen).⠀

🟪A good OTC option is the Skin Renewing Vitamin C Serum. It has a lower concentration of Vitamin C (or L-ascorbic acid) but is a great gentle option. Other OTC products I’ve tried have a grainy texture or don’t rub in quite as nicely. I always try products out myself before recommending to you all!⠀

💟Sunscreen. Retinoid. Vitamin C. Use these three things every day and your future self will thank you!

Perioral dermatitis. I have seen SO much of this rash lately.

▫️What is it?
-a common rash with lots of small red bumps or pustules (pus bumps) around the mouth, nose, and sometimes around the eyes
-can itch, sting, burn, or have no symptoms at all

▫️What causes it?
-using topical steroids (ie cortisone cream) on the face, intranasal steroids, or inhaled corticosteroids
-using occlusive creams or certain facial products
-hormonal changes

▫️Two things that can exacerbate this rash:
STRESS 😫 and increased heat/moisture (MASK WEARING) 😷

▫️How to treat
-reduce stress (ha! Jk, who ISN’T more stressed these days?) 😅
-if using topical steroid creams on the face, SLOWLY reduce usage, then stop; abruptly stopping steroid creams may actually cause the rash to flare
-if using a heavy, occlusive face product, switch to something non-comedogenic for sensitive skin
-a prescription cream (ie triple rosacea cream—> my favorite treatment for this, or other topical prescription creams)
-a short course of oral antibiotics may be needed to clear the rash

▫️What to expect:
-the rash should clear with treatment BUT this rash can recur… hopefully it resolves once and for all 🙌🏼 but maintenance treatment is often required

Swipe to see which lesions were skin cancer 👀➡️⠀

✅Take home points:⠀
1. Sun damage is CUMULATIVE. This patient wasn’t expecting skin cancer because she hadn’t had any sun exposure in recent years, only sun exposure in her younger years.⠀
2. Not as common, but skin cancer does occur in Asian, Hispanic, and Black patients.⠀
3. This patient was most worried about the growth on her forehead, which turned out benign. I spotted other concerning lesions which turned out to be cancerous. This is why it’s so important to have a full body skin exam by a board certified dermatologist. 👩🏻‍⚕️We often spot cancerous lesions you may not even have noticed. We are trained to find skin cancers in ALL skin types so they can be diagnosed and treated as soon as possible!⠀

I see LOTS of molluscum contagiosum all year, but seems I see MORE during the summertime/right after summer.⠀

▫️What are molluscum?⠀
-small, generally painless, skin colored or light pink bumps that range from the size of a pinhead to a pencil eraser⠀
-can be found anywhere on the body but often on the face, neck, arms, or buttocks in children⠀
-caused by a virus called the poxvirus⠀

▫️How are they spread?⠀
-usually by direct skin-to-skin contact (SO common, it’s often difficult to know exactly where your kid got them)⠀
-try not to pick/scratch them off as they can spread⠀
-swimming pools have been associated with infection (probably why I see more during/shortly after the summer!)⠀
-wet skin makes it easier for molluscum to spread⠀
-💡Tip—>have your kids change out of their bathing suits SOON after getting out of the pool⠀

▫️What about treatment?⠀
-in many cases, molluscum will go away on their own within 2 years, so you can just watch and wait!⠀
-BUT, if left alone, molluscum can spread from having only a few to more than 100!⠀
-if you notice more than 5-10, I would treat, as molluscum can leave small scars and discoloration (even with or without treatment)⠀

▫️How do I treat?⠀
-keeping skin moisturized is SUPER IMPORTANT—maintaining the skin’s "barrier protection" prevents molluscum from spreading ⠀
-understand that multiple treatments are often required— molluscum can be quite stubborn!⠀
-treatment methods commonly used: ❄️ liquid nitrogen (aka freezing the bumps), 🧴topical creams, 💊oral pills (which can help “boost” the immune system to fight the molluscum), 🧰curetting them off with a special tool⠀
-I often freeze using a cotton tip— I explain to kids that it feels SUPER cold like 🍦ice cream, but it can sting a little. If they can TRY to stay still, they get to see a special “magic trick” afterwards 🎩🐰😉⠀

▫️Other notes:⠀
-Kids with eczema are MORE susceptible to molluscum and may have more lesions than kids without eczema. We may even focus on treating the eczema BEFORE we treat the molluscum—it’s that important⠀
-Molluscum can become inflamed (red & swollen)—> this is usually a GOOD sign that the body‘s trying to get rid of them!

Talking about one of my favorite (and most frequently prescribed) medications today! Retinoids/Retinols can do so much for your skin. Here are ones I prescribe/recommend most often:⠀

Retinoids: ⠀
-Retin-A⠀
-EpiDuo Forte⠀
-tretinoin⠀
-Differin—>this used to require a prescription but is now available OTC⠀

Retinols: Here are a few options!⠀
-Revision Skincare Retinol Complete (available for purchase in-office)⠀
-RoC Retinol Correxion Deep Wrinkle Night Cream⠀
-Cerave Retinol Resurfacing Serum⠀

PLEASE NOTE: Retinoids/Retinol are not safe during pregnancy. As for use while breastfeeding, there have not been any studies. However, since so little gets absorbed systemically through the skin, it is *probably* safe. I used a retinoid while breastfeeding, but if you are more cautious, just hold off until you’re done!⠀

If you know me, you know that my skin care regimen is pretty basic. Anti-aging skin care can be simple and does NOT have to break the bank to be effective. I emphasize 3️⃣ products because they have the most RESEARCH-BASED EVIDENCE showing that they help prevent skin aging and sun damage:⠀

1️⃣ Sunscreen (I know it seems basic/boring but it is fundamental)⠀
2️⃣ Topical retinoid or retinol (retinOIDs such as Retin-A require a prescription; retinOLs do not, but are not as “strong”)⠀
3️⃣ Vitamin C serum (often a bit more pricey, but WORTH the splurge for a good one)⠀

These 3 items are where you want to spend your $$. Find products that you LOVE and that you will use EVERY DAY. I will share specific products I recommend & use in the future.⠀

As for everything else (ie face washes, moisturizing lotions, etc), if you have an expensive product that makes your skin feel luxurious and you love it, go for it! Otherwise, find a simple product with simple ingredients and don’t feel bad about it. This is where you can save your money.⠀

Three years ago TODAY, we opened our clinic and saw our very first patients at FWDC. Dr. Carroll and I started this practice from scratch. We run things ourselves and our goal is to give our patients the very best care.⠀

▫️If your appointment is a follow up visit for acne, rosacea, psoriasis, or any other rash, we are happy to put you on our tele-derm schedule. ⠀

How many of you know the heart-breaking cry of a baby with a bad DIAPER RASH? 🤚🏼As a mom of two babies with super sensitive bottoms, I've had my share of experience. Here are my diaper rash tips and favorite products to share with you! 🚼⠀

♦️PREVENTION is key!⠀
Change those diapers frequently. Stool and urine are very irritating-- minimize the time that stool/urine are contacting the skin.⠀

♦️Diaper paste--layer it on THICK. Like icing a cake & apply with EVERY diaper change.⠀

♦️Favorite diaper creams-⠀
-Desitin 40% Max Strength (purple box): I buy 3-4 tubes at a time. Note: this contains fragrance, so if your baby is particularly sensitive, use Triple Paste instead.👇🏼⠀
-Triple Paste: another favorite.⠀
👩🏻‍⚕️If over-the-counter creams are not clearing the rash, I like to prescribe zinc oxide compounded with cholestyramine. Cholestyramine can help bind up bile acids in stool and can really help reduce irritation. *I have used this combination to clear up some of the WORST diaper rashes-- it WORKS.*⠀

♦️BabyBum Diaper Cream Brush-⠀
-One of my FAVORITE baby products. I had never heard of it before but received it as a baby shower gift and now I use it with every diaper change.⠀
-It keeps your fingers clean--I dislike the feeling of sticky stuff on my hands and diaper paste is really hard to wipe off!⠀
-$7.99 at Target, BuyBuyBaby, or Amazon.⠀

♦️Baby Wipes-⠀
-Water Wipes- super gentle wipes without a lot of chemicals/ingredients that can irritate the skin.⠀
-You can also skip wipes and use just warm water and a soft washcloth to rinse off (easier said than done).⠀

♦️Wet, moist skin can be a breeding ground for yeast (can see little red bumps/dots outside the main rash) and bacterial infections (staph, strep). Other rashes such as seborrheic dermatitis, psoriasis, nutritional deficiencies (rare) can also present in the diaper area. If the rash does not improve, be sure to see your dermatologist/pedi for other recommendations.⠀

♦️Lastly, give yourself a little grace. It can be so easy to lose track of time. It can literally take just 10 min of my kids sitting on a 💩 diaper for their skin to act up! 🙃

DID YOU KNOW that you can REDUCE your risk of skin cancers and pre-cancers by taking a safe, inexpensive, over the counter supplement called niacinamide?⠀

🔹What is it?⠀
Niacinamide (aka nicatinamide) is a form of Vitamin B3 that can reduce your risk of squamous cell carcinomas, basal cell carcinomas, and actinic keratoses (or "pre-cancers”). It works by enhancing your body’s ability to repair DNA damage from UV exposure.⠀

🔹Is it safe?⠀
YES! There are no known major side effects. Just don’t confuse this with NIACIN which can cause headaches and flushing.⠀

🔹Where can I find it?⠀
Niacinamide is widely available and requires no prescription. You can find it on Amazon or at most local drug stores.⠀

🔹How much should I take?⠀
500 mg TWICE DAILY (as studied in a large, randomized control study published in the New England Journal of Medicine).⠀

🔹Who should take it?⠀
Anyone with a history of skin cancer, extensive sun damage, or lots of "pre-cancers" (ie your dermatologist is treating you with liquid nitrogen ❄️ every time you see them).⠀

👩🏻‍⚕️ I recommend this supplement all the time and patients tell me they like it! Comment below 👇 if you're already taking niaciamide or if you're going to give it a try! ⠀

The cold weather outside + having the heater on inside is already making my skin feel dry!⠀

🔹Here are my FAVORITE moisturizing creams 🧴 that I recommend to patients every single day. You can find all of these at any Target, Walmart, or local drug store.⠀

1️⃣Cerave Moisturizing Cream⠀
-thick moisturizing cream without a greasy feeling⠀
-contains ceramides which are building blocks in the skin to help with barrier function (helps lock moisture into your skin) ⠀
-contains hyaluronic acid which helps our skin retain water⠀
-by far my favorite moisturizing cream 🙌-- I personally use it every day and if you've seen me in clinic, chances are I've recommended it to you ⠀
-$12/jar or you can get TWO jars for $20 !⠀

2️⃣Aveeno Baby Eczema Nighttime Balm⠀
-my go-to for babies and kids with dry skin or eczema⠀
-slightly greasier feel but we rub this on both of our babies every single night after bathing and they don't seem to mind ⠀
-my toddler had mild eczema as a baby and this definitely kept his flares at a minimum ⠀

🔹General tips for managing dry skin:⠀
-SHORT showers or baths lasting less than 10-15 min⠀
-LUKEWARM, not hot water⠀
-use a gentle, fragrance-free soap such as Dove Sensitive Skin Bar Soap or glycerin soap (can find at Whole Foods, $2) ⠀
-can limit soap use to "dirtier" areas on the body such as the axilla, groin, and feet ⠀
-pat dry (no need to aggressively dry) and IMMEDIATELY apply a thick, bland moisturizing cream (such as Cerave Cream!)

Basal Cell Carcinomas are the most common type of skin cancer. ⠀

➡️These are generally very slow growing tumors but if left untreated, they can become quite large or spread beyond the skin (luckily this is very rare).⠀

➡️Appearance:⠀
-pink, "pearly" or translucent bump⠀
-an open sore that does not heal⠀
-may have raised edges or an ulcer in the center⠀
-may ooze, crust, itch, or bleed⠀

➡️This patient had a Superficial Basal Cell Carcinoma which means it only affects the top layer of skin. As you can see, it doesn't always have the classic features of a regular BCC and can be quite subtle. Using a dermatoscope 🔎 helped me identify this skin cancer. Patients often don't even realize that there's something there! 👀⠀

➡️Treatment options include:⠀
1️⃣Excision ⠀
2️⃣Electrodessication & Curettage (ED&C) aka "scrape & burn"⠀
3️⃣Topical chemo/immunotherapy⠀

It’s always a good time to get your skin checked!⠀

When you have lots of spots, it can be difficult to know what’s fine and what’s not. Using my dermatoscope 🔎, I was able to diagnose a malignant melanoma on this patient’s back. Thankfully, melanomas are not as common as basal cell and squamous cell carcinomas but they act more aggressively and can spread through the blood and lymph nodes. If diagnosed and treated early, there is a great cure rate.⠀

I often get asked if I can quickly look at a photo to see if a spot is “okay”. This case highlights the fact that pigmented lesions can appear fairly benign on a photo but are actually cancerous.⠀

It is best to get an IN-PERSON examination where there is good lighting💡and the appropriate tools 🔎.⠀

Our bodies go through SO MANY changes during pregnancy, including our skin!⠀

🌟Highlighting a few here:⠀
*⃣Melasma—darkened patches on the forehead, cheeks, or upper lip. Sun protection is KEY. SPF 30+, preferably TINTED to help protect from even INDOOR light. Melasma may improve after 🤰🏻or can be treated postpartum with topical retinoids, hydroquinone, etc.⠀
*⃣Acne & rosacea—a lot of women get to experience a “pregnancy glow” but others may experience the OPPOSITE with worsening acne & rosacea. Use gentle products as your skin may be more sensitive and prone to break outs. There are both over the counter and prescription meds to treat during 🤰🏻but options are limited! Seek advice from your friendly dermatologist 👩🏻‍⚕️for the best treatment for you!⠀
*⃣Varicose veins—that belly adds a lot of pressure to underlying blood vessels. You bet I wear compression stockings during clinic days to help prevent varicose veins. Keeping your legs elevated will also help relieve some of that vascular pressure. While your feet are up, ask for that much needed foot massage while you’re at it.😉⠀
*⃣Striae gravidarum—oh dreaded stretch marks! Belly, buttocks, thighs, breasts—you name it, I’ve got ‘em there. Up to 90% of women get them. They are due to hormones & stretching of the skin PLUS a genetic predisposition (thanks, Mom 😩). Unfortunately, research has not yet shown anything to adequately prevent stretch marks (although I think using a good moisturizer to those common areas wouldn’t hurt!).⠀
*⃣Postpartum hair loss—although your hair may be more full during pregnancy (👋gorgeous locks), starting about 3 months after delivery, many women notice increased hair shedding. This can last for months and can be quite shocking when it seems you are losing fists full of hair. Rest assured, normal hair growth by your child’s 1st birthday is the NORM.⠀

Lastly, whatever skin changes you may experience, be SO 👏🏼PROUD that you carried and nourished a little 👶 in that body for 9 MONTHS. It's truly such a blessing. Most of these things will improve with time. Until then, MAMAS--wear those marks with PRIDE 👊🏼, and I’ll try my best to do the same!
