In order to understand issues related to breast implant capsules, you have to understand why capsules form in the first place. A breast implant capsule is essentially a foreign body reaction. The body recognizes the implant as a foreign material and tries to contain it by surrounding it in a capsule of fibrous scar-like tissue. Roughly 90% of the time the capsule remains soft, but changes can occur for a variety of different reasons. Fat grafting is an amazing tool that improves both my reconstruction and aesthetic breast outcomes. It begins with liposuction. The fat that is collected is then processed to remove excess fluid and then the fat graft is carefully injected into the breast. When I met this incredibly strong woman she was undergoing neoadjuvant chemotherapy (chemo before surgery). We had a long discussion about options and she choose a bilateral nipple sparing mastectomy and DIEP flap reconstruction with sensory nerve reinnervation. 1 year ago I said goodbye to cancer and my breasts. I can’t believe it’s been 1 year! Some days it feels like yesterday and some days it feels like it was a long long time ago. Never once did I think I was making a horrible decision. Never once did I regret this choice. This was the biggest decision I have ever had to make and my decision alone. Morning of surgery I remember getting ready and saying goodbye to my breasts. I didn’t shed 1 tear because I knew by the end of the day the cancer would be gone. I knew I won! I didn’t give cancer a chance to grow. The only time I broke down was when I was waiting for the nuclear injection for the lymph nodes and I got a call from Abigail and Aaron. I fought back the tears and tried to ignore the thoughts that were rushing through my mind. Is this going to be the last time I hear their voices? Is this the last time they are going to hear me tell them how much I love them? Am I going to wake up when this is over? If I don’t will they remember their aunt? Will they think I was a warrior? Will they know I did this so I could enjoy watching them grow up? I remember hearing Abigail’s sweet loving innocent voice say “Aunt Trish are you crying?” I told her they were tears of happiness because I was so happy they called me to wish me luck and that I love them so much. Here I am 1 year later cancer free and loving my nieces and nephew every second of every day. Having them by my side through recovery gave me the strength to heal and be happy and positive through all of this. I am also so grateful I was referred to NYBRA and Dr. Light. I knew from the moment I met him that I was in the right place and the right hands. Now a year later I get to watch both my nieces and nephew grow up and not have to worry about missing anything. Abigail, thank you for being my recovery buddy and for all your get well notes. I loved having you almost every afternoon. I enjoyed our time together and you helped me get stronger everyday. Aa

#Repost We are very lucky to have @marnie_medicaltattoo putting the final touches on our reconstructions. Women that have had their nipple and areola removed at the time of their mastectomy have 2 options for recreating a nipple. Some women will have me re-create the nipple using their own skin, and then a tattoo placed over and around the nipple to add further detail to the nipple reconstruction and provide the pigmentation for the areola. The second option is a 3-D nipple tattoo. In these cases Marnie Rustemeyer, our tattoo artist, will perform a nipple areola tattoo which appears 3 dimensional but has the advantage of being flat. This lovely woman choose to undergo a left mastectomy to treat extensive DCIS. It was important for her to preserve her right breast in order to retain full sensation. ⁠ ⁠ Trying to match a natural breast to an implant reconstruction can be challenging. Most of the volume in a natural breast is below the nipple, where a implant reconstruction is closer to an 50/50 split, half above and half below. Luckily, this patient desired to increase her breast volume. This allowed me to perform a left breast reconstruction to her desired size and then place a smaller breast implant in the right breast along with a breast lift to improve symmetry.
