Happy National Doctors Day .davidlight! There are no words to describe how appreciative I am to have you as my doctor. You made my journey through reconstruction worry free. I am not good with visualizing things. I had a hard time visualizing how I would look at the end of all of this and let me say I look better than I could ever have imagined. Hearing the words you have breast cancer is devastating no matter what stage. I can’t even remember everything we discussed during my first consultation my mind was all over the place. But the one thing I knew for sure was that I was in the right place the right surgeon. I remember leaving and my parents asked me what I thought and if I wanted to look for another surgeon that was in network. I said no I’m staying with Dr. Light. I just felt the sense of comfort from the moment I stepped into NYBRA. I am so happy I stuck with my gut feelings with him. Dr. Light cares about each and everyone of his patients and goes above and beyond for us. He has such a calming aura which made going in for surgery easy. Today was the second male chest wall reconstruction I performed this month for male breast cancer. For men, there is no screening available and most are detected on physical exam. The challenge in men is that many do not realize they have breast tissue. Lumps in their chest are often ignored and allowed to grow.⁠

Men with genetic mutations, such as BRCA, have an increase in their lifetime risk for breast cancer. Genetic mutations for breast cancer are more often found in women, given the higher incidence of breast cancer in women. It is important not to ignore the men in a family when a mutation such as BRCA is found. Men should be tested and cancer surveillance should be increased if they test positive. Some men also opt to undergo prophylactic mastectomies.⁠

An aesthetic flat closure is an important option that should be discussed with women planning on undergoing a mastectomy or explant surgery. During an aesthetic flat closure, excess breast skin and tissue is removed so that the chest wall skin heals with a smooth, flat appearance. Depending on the patient’s breast size, different incision patterns are used to prevent contour irregularities and “dog ear” deformities. ⁠

Aesthetic flat closures can be performed at the time of a mastectomy, following an explant surgery or for patients that have previously undergone a mastectomy but are not happy with the contour/appearance of their chest wall. ⁠

Thin patients can still be good candidates for a DIEP flap. This woman's goal was to be about the same size as her natural breasts. She absolutely did not want an implant reconstruction. Despite being very fit, she still has some excess fat in her lower abdomen. She also had a re**us diastasis (weakness and spreading of the tissue between her 6 pack muscles). ⁠

She underwent a bilateral ni**le sparing mastectomy using a hidden scar under the breast (inframammary), DIEP flap, Resensation procedure and diastasis repair. Now she has a warm natural reconstruction similar in size to what she was used to and a flatter, stronger abdomen after her diastasis repair.⁠

Welcome to DIEP Week!! Stay tuned this week as we raise awareness for one of the most commonly performed natural tissue breast reconstructions, DIEP flaps. Changes to medical billing codes may threaten the ability for surgeons to offer DIEP flaps to their patients. Not every patient may be a candidate or want a DIEP flap, but every patient should have the option!⁠

If you can tell me what the arrows are pointing to you are already on the fast track to an A+ in DIEP flaps 101. That's the MR angiogram for my patient tomorrow. Can't wait!!!⁠

This petite woman in her mid 20's came to me interested in a breast reduction. She had 34DD breasts which caused her neck and back pain. She also complained of rashes underneath her breasts, especially during the summer months. This is a classic presentation for women with macromastia (large breasts).⁠

She underwent a 400 gram gram breast reduction. and reached her goal of becoming a B to C cup. Her breast reduction was also successful in eliminating her neck/back pain and rashes. I'm thrilled that she is happy and can get back to the activities that she loves.⁠

Yes!... breast reductions are often covered by insurance. Reductions are considered medically necessary when a functional impairment, such as back and neck pain, occurs due to the patients breast size. There is a process for obtaining authorization from your insurance company which begins with a consultation during which photos are taken and an examination is performed. ⁠ At the end of the consultation you will meet with our authorization department who will handle your approval process from start to finish.⁠

I've said from the beginning that breast cancer was going to be one of my biggest blessings, so it was no mistake that God placed one of the GREATEST blessings and most amazing surgeon .davidlight in my life. He is by far one of my favorite humans who has gone above and beyond for me not just physically putting me back together, but emotionally. This man has talked me off the ledge more than once and has calmed me just by his kindness and compassion in knowing what I was dealing with. I'm not sure I will ever be able to thank him for making me feel whole again and out doing himself on the work he's done on this body...these foobies are keepers and I am absolutely adopting them this time around. 🤣

My experience with NYBRA Plastic Surgery Group in Great Neck, New York.
There are reasons people come from all over the Country for the incredibly gifted surgeons in this office. Let’s back track a bit though. When you call to make an appointment, you actually get a real person on the phone. A real nice person too!! Everyone is compassionate and a pleasure to speak with. Then when you get to their office, there may be a wait. Not a long wait however, a reasonable wait. The Doctors take time with their patients. They understand the decisions we must make are not easy ones and require time to explain. You will get the time you need. This office is also one of a kind because they have a very special Lady, Mollie Sugarman. Mollie runs a Patient Empowerment Program. So much support is offered here. Dr. David Light is my doctor. I chose an AFC. I didn’t have reconstruction because of other medical issues. I have BRCA2. This was my choice and I have a surgeon that had my back with my decision. I got the respect I needed and the support from Dr Light. He is a compassionate and gifted Surgeon as are all of the Doctors in this practice. Yes my feeling is NYBRA is where I felt safe and supported having my Double Mastectomy. Judging by how busy the office is, I’m not the only one that feels this way❤️

Not every patient who wants a natural tissue reconstruction is a good candidate for a DIEP flap. Some may have had a previous tummy tuck, others are just naturally too thin in the lower belly. For these patient, we look to alternate areas like the inner thighs TUG/PAP flaps) or lower back (LAP flap) for suitable tissue for their reconstruction.⁠

For this amazing woman in her 50's, I turned to her inner thighs and performed a Fleur de lis TUG flap to complete her reconstruction. Similar to a DIEP flap which improves the abdominal contour by performing a modified tummy tuck, the TUG flap improves the inner thighs by performing a modified thigh lift. ⁠

This lovely 57 year old woman was previously diagnosed with a right ductal carcinoma and opted to undergo a bilateral ni**le sparing mastectomy and DIEP flap reconstruction.⁠

This was a single step reconstruction. The patient was happy with her warm, natural breast reconstruction and did not undergo a revision procedure.⁠

So thankful she trusted me with her care!⁠

We are excited to share an upcoming project with NYBRA patients, Dr. David Light and Mollie Sugarman, Clinical Director of our Patient Empowerment Program (PEP).

This discussion will focus on the effects of a cancer diagnosis on single women under 40 years old. While many of the issues are those that all our patients confront, some are specific to this particular population.
