Had a great weekend with the family in Birmingham while catching up with great friends and colleagues at the ASMI Blind Squirrel Society Meeting! Had the opportunity to see Dr. Andrews and Dr. Cain interview coach Nick Saban in a fireside chat. It was also Mia's first plane ride and she did great!
π¦΅Can the ACL heal? With newer technology and advanced techniques there has been growing interest in ACL repair rather than reconstruction.
During repair the patient's ACL tissue is repaired back to the tear site and augmented with a high strength suture called an internal brace ().
Picture 1 shows an arthroscopic photo of the ACL where the tear is unclear.
Picture 2 shows that on further inspection the ACL is torn from it's origin on the femur.
The ACL tissue is sutured (picture 3) and repaired back to its naitive location on the femur with the addition of the Internal Brace (red arrow) which is the blue suture laying over the ACL (pictures 4 and 5).
Is ACL repair a good option for you? ACL repair is not possible in all tear types and all patients. But for the appropriate tear, with good tissue quality, in the correctly indicated patient it is a great option!

Another happy patient after arthroscopic meniscus repair! This patient had a repair of the meniscus root, a very important structure for joint preservation to prevent arthritis.
After a full recovery he is back running and playing sports without pain and doing great!
π¦΅π€ Knee pain from arthritis affects hundreds of thousands of Americans a year. In highly active people with arthritis and pain only in one compartment of the knee a partial or "unicompartmental" knee replacement may be an excellent option.
This is performed using a CT scan for 3D mapping of the patient's anatomy and with the Mako surgical robot. The robot allows for perfect removal of the arthritic bone and placement of size matched implants.
π€ Picture 1 shows the robot used during surgery.
Picture 2 is an actual case of mine in an active male with isolated medial joint space narrowing and pain. He failed nonsurgical management and underwent robotic-assisted partial knee replacement.
ππ½ Pictures 3 and 4 show post op Xrays of the medial compartment replacement performed outpatient where the patient was discharged home just hours after surgery!
ποΈββοΈπ₯π€― Lifting something and feel a POP in your elbow? One possibility is a tear of the biceps tendon at the elbow. Patients with this injury develope pain, deformity and bruising from the tear and retraction of the biceps muscle (picture 1 and 2).
Surgery is recommended in healthy active people to prevent weakness, chronic pain, and cramping!
π· Through a minimally invasive 3cm incision the tendon is repaired back to the bone utilizing a suture button. Picture 3 shows the tendon out of the wound prepped with our repair sutures. Picture 4 is after anatomic repair to the biceps tuberosity! πͺ
My patients are placed in a sling for comfort for 4 weeks and begin immediate motion after surgery. Strengthening begins at week 8 and most patients fully recovery by 4-6 months post-op!

Meniscus root tears in young athletes need to be fixed to prevent downstream arthritis. Studies have shown that a tear at the root attachment increases contact forces to near total meniscectomy (meniscus removal).
Picture 1 shows the tear at the root attachment (yellow arrow)
Picture 2 shows how the root can be lifted away from the bone to identify the tear.
During surgery we drill a small tunnel at the attachment and place multiple repair sutures around the meniscus root as seen in pic 3.
We then shuttle those sutures through the tunnel to re-attach the meniscus to it's anatomic location (pic 4)
Picture 5 shows how the meniscus can no longer be elevated away from the bone now that it is anatomically repaired!
