If you have questions or need to enroll you can call the Enrollment Broker: 833-870-0550 (toll free).

If you do not select a plan then they will auto enroll beneficiaries starting May 15, 2021.

💉 FLU VACCINES HAVE ARRIVED💉 We will offer flu clinics Tuesday & Thursdays 10-12 and 2-4:30. Please call to schedule an appointment as our waiting room is closed. Flu consents are available on our website. We have a limited number of state vaccines, if your child has Medicaid please call the office to make sure they are available.

We will implement a new appointment reminder system this week. Patients will receive reminders one week prior to their appointment as well as a 24hour reminder. You will receive a phone call and or a text message. Please make sure we have your correct contact information on file.
