The Behavioral Health Specialist identifies and manages clients with behavioral health problems within the primary care setting. Facilitate intake and situation-specific evaluation and referral services for clients in a community mental health setting. In addition, the behavioral health specialist provides skill training through psychoeducation and patient education strategies and develops specific behavioral change plans for clients and behavioral health protocols for target populations.
Minimum Qualifications & requirements:
- Masterâs degree in Psychology
- 2 years experience as Behavioral Health Specialist or other related health field.
- Must complete Mastercareâs pre-employment and new hire process.
- Must possess a positive, enthusiastic attitude, good people skills, and professional appearance.
- Ability to speak, write and read the English language.
- Able to establish and maintain healthy working relationships with clients and employees.
- Able to deliver effective results, meet tight deadlines and targets.
- Strong organizational and communication skills.
- Good judgment and decision-making skills.
- Ability to work with standard office equipment including, computers, phones, printers, faxes
- Proficient in Microsoft Office
Physical Demands:
- Must be physically able to perform the duties of the position, including lifting a minimum of 50 pounds with or without accommodation.
- The position requires frequent bending, stooping, crouching, sitting, standing, walking, pushing, pulling and any movements associated with a normal home care client.
- Must be able to see and read printed materials.
- The position requires the ability to communicate and interact with clients, management, and care associates, and must be able to exchange accurate information in these situations.
Work Environment:
- The Behavioral Health Specialist will typically work in an organized professional office environment but may also work in various client homes, with varying conditions and elements. There is exposure risk to disease, blood borne pathogens and bodily fluids. Personal protective equipment is required for all personal care procedures when caring for clients.
Essential Duties:
- Conduct intakes, interviews, and screenings of prospective clients.
- Conduct assessment of clientsâ addiction behaviors, need for socialization and other life changes.
- Recruit DD/ID clientele.
- Write a behavioral care plan under supervision of a licensed psychologist.
- Working closely with clients to develop treatment plans and make referrals as needed
- Evaluates participant outcomes and or progress toward achieving the objectives and goals of the care plan and communicates this information among other members.
- Assists in the detection of at-risk patients and development of plans to prevent further psychological or physical deterioration.
- Assists in the detection of at-risk patients and development of plans to prevent further psychological or physical harm.
- Safety Issues including, but not limited to: symptoms of abuse, neglect or exploitation; suicidal/homicidal ideation; psychiatric decompensation.
- Maintain complete chart notes; maintain confidentiality and adhere to all HIPAA policies.
- Educates and counsels the participant, or caregiver regarding care needs, options and other related problems. Ability to manage routine office files.
- Any changes in the Care Plan are appropriately communicated
- Scanning files, document management, and maintenance of client files
- Help promote our business through marketing support, such as, job fairs, senior fairs and community involvement
- Extensive communication, both verbal and written, with employees, clients and management
- Other duties necessary and required
Urgently hiring PAB workers (personal assistant habilitation)
Starting rate $15-19/ hr
Minimum Qualifications:
-Must be at least 18 years of age
-One (1) year experience in: Hospital, LTC facility or home care within the last two (2) years
Must complete Mastercareâs pre-employment and new hire process
Pass a physical examination after being offered employment and health assessment annually thereafter
Must be able to read, write and speak English. (Some employees may be hired to care for another Nationality will be required to read, write and speak for the Nationality for which they are hired i.e. Japanese Clients)
-Must be able to follow direction and accurately report to the Field Supervisor an changes in the clients condition
-Must be able to display a caring attitude toward clients and their medical needs
-Must have available, reliable transportation to and from assignments
-Must be physically able to perform the duties of the position, including lifting unspecified weights depending on the assignment requirements, bending, stooping, sitting, standing, associated with a normal home care client
-Must maintain current credentials, including CPR, First Aid, TB Test, Physical, Fingerprinting, In-services (if applicable)

-Assists in grocery shopping as needed

Essential Duties:
-Knowledgeable of the individualized Care Plan for clients, and provides support to clients according to their Care Plan
-Documents services provided in an accurate and timely manner in accordance with agency policy
-Report changes in the clientâs condition or behavior to the RN Supervisor in a timely manner
-Notifies Nursing Supervisor when clarification or education of a task is required
-Fully understands all aspects of the clientâs rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the RN Supervisor or management incidents or evidence of client abuse or violation of clientâs rights
-Attends to the individual needs of clients, which may include assistance with grooming, bathing, oral hygiene, meal preparations, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating or other needs in keeping with the clientâs care requirements
-Assists in maintaining a safe, clean, and healthy environment through light housekeeping including changing bed linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry
Performing other housekeeping tasks as indicated in the Care Plan
-Assists in grocery shopping as needed
Assists and es**rts to and from appointments / errands
-Assists and es**rts to recreational events & outings. Participates in activities and functions as directed
Takes clientâs vital signs and recording input and output
-Communicates and interacts effectively and tactfully with clients, families, supervisor and other employees
Work effectively as a team contributor on all assignments by providing specific information and observations of the clientsâ needs and preferences
Submits timecards/ activity sheet in a timely manner (i.e. weekly)
Implements appropriate safety and infection control practices
-Maintain patient confidentiality at all times as required by HIPAA regulations
Other duties as necessary or required

Position Summary:
Under the direct supervision of the Mastercare Service Supervisor, the PAB 1 is responsible for the care of a registered Mastercare DD (developmentally disabled) client in the privacy of the clientâs own home. Caregiver will visit the clientâs home and perform quality, compassionate care within the scope of the clientâs Individual Service Plan (ISP). The Developmental Disabilities (DD) clients are serviced through Medicaid insurance under a program called PAB Waiver service. PAB=Personal Assistant Habilitation.
Minimum Qualifications and Requirements:
- Must be at least 18 years of age
One (1) year experience in: Hospital, LTC facility or home care within the last two (2) years
- Must complete Mastercareâs pre-employment and new hire process
Pass a physical examination after being offered employment and health assessment annually thereafter
- Must be able to read, write and speak English. (Some employees may be hired to care for another Nationality will be required to read, write and speak for the Nationality for which they are hired i.e. Japanese Clients)

All workers are referred as Direct Service Workers (DSW), but will break down to different skill levels according to which client they are servicing. The following job description is a generalization of duties that fall under each level.
