Many healthcare providers have been trained to recommend the RICE protocol (Rest, Ice, Compression, and Elevation) for orthopedic injuries. However, recent research has led to a shift in our understanding of the best approach to injury recovery. In fact, it turns out that two components of the RICE protocol, ice and non-steroidal anti-inflammatory drugs (NSAIDs), may not be as effective as we thought, and could even delay healing. In this newsletter we'll dive into a new protocol that is gaining popularity among healthcare providers: PEACE & LOVE.
Ice and NSAIDs are commonly used to reduce pain and swelling associated with injuries. However, studies have shown that these methods can actually hinder the body's natural healing process.
While ice can provide temporary pain relief, it also constricts blood vessels, which limits blood flow to the affected area. This reduced blood flow can impede the delivery of essential nutrients and oxygen needed for healing.
NSAIDs, such as ibuprofen and aspirin, are often prescribed to alleviate pain and reduce inflammation. However, research suggests that NSAIDs can interfere with the body's natural healing mechanisms by blocking prostaglandins, which play a crucial role in the repair process. Prostaglandins promote inflammation, but they also stimulate the production of collagen, a vital protein for tissue repair. By blocking prostaglandins, NSAIDs may unintentionally slow down the healing process.
In light of these findings, a new protocol has emerged: PEACE & LOVE. Let's break it down:
Protection: Restrict or unload movement for 1-3 days to prevent further damage. This allows the body to begin its natural healing process without additional stress or strain on the affected area.
Elevation: Elevate the injured limb above the level of the heart to reduce swelling and promote blood flow. This helps to remove waste products and bring nutrients to the damaged tissues, supporting the healing process.
Avoid anti-inflammatories: Avoid using anti-inflammatory medications, such as ibuprofen or aspirin, as they can disrupt the body's natural healing processes.
Compression: Use elastic bandages or taping to compress the affected area, helping to reduce swelling and provide support. Be careful not to wrap too tightly, as this can cut off circulation and hinder the healing process.
Education: Educate yourself and others on proper treatment and management of soft tissue injuries. Understand the healing process, load management strategies, and appropriate exercise modifications to ensure a safe and effective return to activity.
Load Management: This refers to the principle of gradually increasing the intensity and duration of physical activity to allow the body to adapt and recover properly.
Optimism: This emphasizes the importance of maintaining a positive attitude and mindset during the recovery process, as research suggests that optimistic patients tend to have better outcomes and faster recovery times.
Vascularization: This refers to the promotion of blood flow and circulation to the affected area, which helps to deliver oxygen and nutrients to facilitate healing.
Exercise: This component highlights the significance of incorporating appropriate exercises and movements into the recovery plan to improve strength, flexibility, and range of motion.
It's time to rethink our approach to injury recovery. The RICE protocol, once considered a cornerstone of treatment, may not be the most effective method after all. By limiting blood flow and potentially slowing down the healing process, ice and NSAIDs might do more harm than good. Embracing the PEACE & LOVE protocol allows us to work with the body's natural processes, enhancing healing and minimizing the risk of complications.

Rhabdomyolysis is a potentially life-threatening syndrome resulting from the breakdown of skeletal muscle fibers with leakage of muscle contents into the circulation. The most common causes are crush injury, overexertion, alcohol abuse and certain medicines and toxic substances.
The “classic triad” of rhabdomyolysis symptoms are: muscle pain in the shoulders, thighs, or lower back; muscle weakness or trouble moving arms and legs; and dark red or brown urine or decreased urination. Keep in mind that half of people with the condition may have no muscle-related symptoms.
