To My Dear and Valued Patients:
It has always been my pleasure and privilege to provide you and your family the most optimal orthopaedic and sports medicine care for your musculoskeletal injuries and conditions. For the past 26 years, I have done so predominantly at Holy Name Medical Center, Hackensack University Medical Center, and most recently, Atlantic Health System in Paramus.

As of October 31, 2022, I have accepted the position in the Orthopaedic Sports Medicine Department with RWJ Barnabas Health System.

My phone number, (201) 343-1717, fax number, (201) 343-3217, and website, https://universityorthopaedic.com will remain the same. My new addresses will be at multiple locations for your convenience:

St. Barnabas Medical Center Ambulatory Care Center:
200 South Orange Avenue, Suite 230
Livingston, NJ 07039
(973) 322-7005; fax: (973) 322-7049

Jersey City Medical Center at Newport:
110 Towne Square Place, Suite 208
Jersey City, NJ 07310
(201) 716-5850; fax: (201) 915-2424

Jersey City Medical Center Medical Office Building:
377 Jersey City Avenue, Suite 550
Jersey City, NJ 07302

Bergen County Office: [Under Construction: Opening December 2022-January 2023]
130 Kinderkamack Road, 3rd Floor
River Edge, NJ 07661
(around the corner from my old office at 433 Hackensack Avenue)

For all those patients in Bergen County, I will continue to remain on staff at both Holy Name and Hackensack University Medical Centers for all your orthopaedic needs.

This position will also bring many additional benefits and resources to you. RWJ Barnabas Health is the largest and most comprehensive non-profit healthcare system in New Jersey, bringing the best of academic medicine, research and teaching, and community partners together to create healthier communities.

This new move will also require a change to the most current electronic health record platform, Epic. This innovative new platform will improve efficiency and safety in your care delivery by providing you and your physicians with your full and up-to-date medical records across institutions. Please be patient with our requests to update your medical records, as well as, to complete patient medical information release forms upon your arrival at my new offices.

Regarding your health insurance policies, I will continue to accept the same policies that I had previously, and a few others will now be included. Any written correspondence sent to you stating I am no longer a participating provider for certain insurance carriers is simply not true. I would still honor all my patients’ relationships with their insurance carriers, regardless of if they are employed through Holy Name Medical Center and Hackensack University Medical Center.

Thank you for your cooperation and loyalty during this transition. If you have questions, feel free to give us a call. My staff and I look forward to seeing you in our new office soon.

Sincerely,

Raphael SF Longobardi, M.D., F.A.A.O.S.
