Our eight incredible Semester in Wilderness Medicine students are now in Montana after an intensive month in Costa Rica. Living in the stunning jungle community of Mastatal, these students resided at Rancho Mastatal, a unique education center rooted in environmental sustainability. After weeks of grueling study and enduring real life scenarios leading them the path of becoming licensed EMTs, these students were joined by physicians, nurses, paramedics, and other EMTs to care for over 150 local patients at our annual pop-up clinic. We LOVE being able to provide this unique care for this community year after year. Please join us in celebrating these students accomplishments down in Costa Rica and wish them luck this next month! 📝 Field note from one of our Fall SWM Instructor’s, David Kerner (@david_kerner): 🏔️ We are out of the field a day early due to an atypical storm for this time of year dropping several feet of snow on the high passes. Coupled with a mass migration of sheep herdsmen taking the valley camp spots we were out of options to stay to our target date... The students traveled through lush river valley to the high alpine to encounter a legendary storm. We took lightning positions at 12,000 feet and saw snow rates of 4 inches an hour. The students learned how to rewarm in the field, discussed leadership intensely, adopted LNT best practices, and worked through medical scenarios at all time of the day. They crushed it!” 🩺 After an obviously epic trek, the students concluded their program back at the Hanifl Centre. They were guest visitors at a local hospital and got to observe and participate in patient care first hand. 🪶 The students are now off into the real world with a once in a lifetime student experience and a portfolio full of certifications. From running rivers to packing stubborn mules, anterior shoulder dislocations are associated with an array of outdoor pursuits. Our approach to this injury involves a detailed assessment and thoughtful consideration of patient and situation variables. If the decision is made to attempt field reduction, we recommend passive techniques that facilitate fatiguing the involved muscle groups allowing for a relatively spontaneous reduction as opposed to pulling or manipulating the humerus back into position. It’s not that such techniques are wrong, but we’re operating without the benefits of imagery, and our students may very well be performing the procedure for the first time.
