Service Coordinators (SCs) shared how they have used the Integrated Supports Star. 2 different SCs shared that they worked with a man who had been homeless for a long time and helped them find their own place to live. Both SCs shared that the mens’ outcomes were similar: to continue living in their own place and to keep/maintain their place. By using the Split Star to brainstorm current and future supports, they identified possible resources, empowering the men to see that they really can get supports so they can live and stay in their own place. Their OWN HOME and HOPE!
