The question that I receive often from you guys and that I have never discussed here is – How can I get rid of skin discoloration?
First, keep in mind that hyperpigmentation does not always fade. Even with treatment, some hyperpigmentation will be permanent. Without any treatment at all, it can take 3 to 24 months to see improvement. Hyperpigmentation can be caused by two things – hormones and sun damage.
You may also have two types simultaneously.
Hormonal pigmentation is stimulated from contraceptive pills, pregnancy and hormone replacement therapy. It usually can be found on sun-exposed areas of your face including:
- Cheeks
- Upper lip, sometimes creating a moustache appearance
- Forehead
- Bridge of the nose
Most people who have hormonal skin pigmentation tend to have a more olive or dark complexion. Melasma cannot really be cured, but it can be treated so its effects are minimized.
How do we address this type of pigmentation?
1. Oral medications or Lightening creams-
Your dermatologist may prescribe medications or topical treatments to help lighten melasma. Doctors often prescribe bleaching agents, such as hydroquinone, which works by blocking melanin production. While products with hydroquinone can be purchased over the counter, they should only be used under a doctor's care — and only on the darkened areas of the skin. Other common ingredients found in lightening products besides hydroquinone include: licorice extract, N-acetylglucosamine, vitamin B-3 (niacinamide). You should try those if you are patient because those option will take the longest.
2. Retinoids- Derived from vitamin A, retinoids are among some of the oldest over the counter skincare ingredients used. Their small molecular structure allows them to pe*****te deep into the skin and treat the layers below your epidermis.
Retinoids can come in either a prescription or over the counter formula. However, OTC versions tend to be weaker. If you do not see any results after a couple of months, talk to your dermatologist about the prescription retinoid tretinoin (Retin-A).
It is also important to note that retinoids are more often used to treat wrinkles than hyperpigmentation. This means that retinoids may not be the best first-line treatment
3. Chemical Peels- A chemical peel uses acids at stronger concentrations to treat the desired area of skin. They reduce the appearance of hyperpigmentation by removing the epidermis. Deeper versions may also pe*****te the middle layer of your skin (dermis) to produce more dramatic results.
Although many chemical peels are available over the counter (up to 10%), you might consider getting a professional-grade peel at your Esthetician (from 10 to 30%) and at your dermatologist’s office from 30% to 75%. These are more powerful, and they yield quicker results. Usually, you need a series of 6 treatments to see a faster result. Be sure to choose a reputable Esthetician or a Doctor because chemical peels require a very good training. Never get peels from Amazon, especially if they are very concentrated and try to do at home treatment! When used improperly- blisters, scars and bigger areas of hyperpigmentation or hypopigmentation may also develop. Chemical peels work best for fairer skin tones but can be tricky for people with Fitzpatrick skin types IV-VI, especially among Hispanics, African Americans, Africans and Asians Asian or African- American background
4.Laser peel (skin resurfacing)- laser peel treatment uses targeted beams of light to reduce hyperpigmentation.
There are two types of lasers: ablative and non-ablative. Ablative lasers are the most intense, and they involve removing layers of your skin. Non-ablative procedures, on the other hand, target the dermis to promote collagen growth and tightening effects.
Ablative lasers are stronger, but they may cause more side effects. Both destroy elements in your skin to ensure that new skin cells grow back tighter and more toned.
Who should try this?
There isn’t a one-size-fits-all approach to skin resurfacing. Ablative lasers may work better for people with fair skin. For some people, non-ablative versions may cause the skin to darken instead of lighten. Your dermatologist will work with you to assess your discoloration and overall skin tone to select the best option for your skin
5. Intense pulse light therapy (IPL)
IPL therapy is a type of non-ablative (fractional) laser treatment. Also known as a photo facial, IPL therapy stimulates collagen growth within the dermis. It usually requires multiple sessions.
IPL is used for overall pigmentation issues, but flat spots especially respond to this treatment. It may also help reduce the appearance of wrinkles, spider veins, and enlarged pores.
6. Shut down the sun-The next step in treating melasma is to prevent the sun from aggravating the condition. The most important way to clear up melasma is by using a strict sunscreen regimen. But keep in mind that not all sunscreens are created equal. There are two main types of sunscreens:
-sunscreens that use chemicals, such as oxybenzone
-sunscreens that use physical blockers, such as zinc and titanium dioxide.
You want to choose the non-chemical, blocking sunscreen, because that will stop all the light and different wavelengths from coming through. Chemical sunscreens do not offer the same protection for melasma, and in some instances, they may even trigger allergic reactions that can make melasma worse.

How we address the Sun Pigmentation?
1. Microdermabrasion- Microdermabrasion is an in-office procedure used to treat hyperpigmentation that affects the epidermis only (superficial pigment only).
During the procedure, your esthetician or dermatologist will use a drill-like handheld tool with a diamond brush or other abrasive attachment. The tool is then swiped across your skin to rapidly but gently to remove the epidermis. You may need multiple sessions to achieve your ideal result but at some cases you will not be able to remove the sunspots completely. You may have to combine the treatment with chemical peel to achieve the ideal result.
2.Cryotherapy- removes sunspots and other skin lesions by freezing them off with a liquid nitrogen solution. Nitrous oxide may be used (instead of liquid nitrogen) for the treatment of superficial dark spots, such as sunspots, because it’s not as aggressive and is less likely to cause blistering. Cryotherapy takes just a few minutes and is generally well-tolerated.
3.Laser resurfacing. During laser resurfacing, a wand-like device is used to deliver beams of light that remove sun damaged skin layer by layer. New skin is then able to grow in its place. Laser resurfacing on the face can take anywhere from 30 minutes to two hours depending on how many sunspots are being treated. Healing typically takes anywhere from 10 to 21 days.
4. Intense pulse light (IPL). IPL uses pulses of light energy to target sunspots on the skin. It does this by heating and destroying the melanin, which removes the discolored spots. An IPL session usually takes less than 30 minutes and causes little to no pain. The number of sessions needed varies from person to person.

Finally-What can I offer you in my office?
I am not only offering Microdermabrasion and Chemical Peels but also Oxygenation brightening treatment using PURE Oxygen Infusion Portable Machine. The best results are seen if the client is scheduled twice a week for three weeks. The machine uses 21% of oxygen (the same as we breathe). Brightening oxygen infusion serum combats pigmented skin changes that may occur due to sun exposure, medications or hormonal challenges. Due to its anti-inflammatory ingredients, the brightening serum may be used even on hyper-sensitive skin. The serum has Kojic acid, Licorice and Bearberry extract to improve pigmentation concerns without redness or skin irritation.
Another unusual treatment for reducing pigmentation I offer is Red LED-How Red Light Therapy Repairs Sun Damage?
Red light therapy uses specific wavelengths of red and near-infrared light to activate the body’s natural healing processes. When it comes to sun damage and other skin-related concerns like scarring, hyperpigmentation, and wound healing, red light therapy increases the body’s production of collagen and elastin, as well as an increased immune response that helps clear away dead cells and repair damaged ones.
