PANCAKING

I have been asked by many ostomates what to do about an experience that has been described as pancaking. Pancaking is when body waste is being compressed against the inside or outside of the wafer that attaches the pouch to the abdomen of an ostomate. Pancaking is more common with a colostomy then an ileostomy because the stool is more formed and firm. The ileostomy is more liquid and less inclined to develop pancaking.

Pancaking occurs when the pouch is held tight to the body as a result of clothing, a girdle or anything that compresses the pouch against the body to prevent it from creating a bulge on the outside of clothing worn. The body waste comes out of the stoma and has no place to go so it becomes compressed against the wafer that attaches the pouch to the body (abdomen).

The expelled waste will seek the path of least resistance and can go between the wafer and the abdomen or between the wafer and pouch. When this occurs, cleaning the waste becomes a difficult task and can often results in the wafer leaking and the necessity of replacing the entire pouch system. Understanding the cause can help avoid pancaking.

Tight clothing, a belt or anything that causes pressure against the outside of the pouch forcing it against the body can cause pancaking. Sleeping on one’s stomach can also cause pancaking. Controlling the tight clothing or other belts that may compress the pouch against the body can be adjusted, however sleeping on the pouch requires other steps to avoid pancaking.

Leaving gas in the pouch can create a pocket of air between the wafer and the pouch which will prevent the waste from becoming compressed against the wafer during sleep. Using a straw or small hose, air can be blown into the pouch before closing to create a similar pocket of air to prevent compression of expelled waste.

The stoma has no sphincter muscle like the re**um to control flow so waste comes out when the body expels it. The pouching system contains the expelled waste for later disposal. All pouch systems are affected as previously described and the following is a quick reference guide to alleviate this problem:

1. Avoid clothing, girdles or belts that apply pressure against the pouch. Applying
pressure to the outside of the pouch will cause compression of the waste
resulting in pancaking.

2. Add air to the pouch before sleeping to create a pocket that will prevent
compressing body waste, thus creating pancaking. The air creates a pocket
that prevents compression the waste.

3. Leaving gas in the pouch can also create an air pocket that will prevent
compression of waste creating pancaking. The gas also creates a pocket that
will prevent compression of waste into pancaking.

Knowing the underlying causes of pancaking and applying the preventative measures noted above will help to eliminate pancaking from occurring.
