Painless Microscopic Root Canals using latest 3d technology!
In school we were taught if a root canals fails we need to either 1. Retreat 2.apico 3. Extract. We were not really taught to think outside the box 📦. I will post a video that shows outside the box thinking.
Where is the real apex? Sometimes when there is a dead stop there is an exit somewhere.
We finally got a 98 percent save!!! And that we did. Treated 31 2 years ago. Patient wanted to wait on 30. Treated 30 1 year ago and now we have full healing! Root canals work!
This is a great case to share. Number 3 was treated endodontically. Lesion persists apical to number 3 db root. Fistula traced to 3 db root. Asked to see what we should do. 48 percent said to apico 3 db root and 52 percent said to treat 2 endodontically. I really was hnsure of the best treatment for this patient. I decided to treat number 2 as it was non vital and had periapical lesions and needed treatment anyway. Look at that mb puff leads directly from 2 to 3!!! Sometimes we question if teeth are connected. Here we see clearly there is a connection from 2 to 3. Please share this case as it is very telling for future difficult diagnostic cases.
Why do we need 3d imaging in endodontic diagnosis? Look how many of us(including mysef) want to do treatment based on 2 dimensional imaging.
This is a straight forward case to save a tooth! 15 year follow up with complete healing after db root apicoectomy and mb root amputation. Almost 20 percent of people wanted to take this out!
55 percent want to extract this 25 mm lower canine. Saving teeth is still an option.
Once again is cbct or 3d imaging important in endodontic diagnosis? From 50/50 to almost 90/10. Thats a major difference in treatment. We need to see what we are treating!
This is a great discussion. The apico retreat was almost 50/50. Its interesting that we arent closer to 90/10 on these cases. We did the apicos on 3 and 14 on the same week in 2019. Here is a 2 year follow up with complete bone fill without using any grafting material. Is grafting and membrane important for apical surgery? Why would almost 50 percent want to retreat these? Pls share and discuss.
This was a case where the patient chose to retreat an old root canal knowing we may need to apico in the future. Even though we were able to get to length the case failed and an apico was needed. Here is a 1 year recall and full healing. Don’t give up if a case fails.
This happens to be one of my favorite cases. Think outside that box 📦!! 81 percent wanted to extract this tooth. It is not wrong. Im sure many of you think this is crazy. But patient was motivated to keep his tooth. Lets share and discuss outside the box thinking. Here is a 4 year recall and a very happy patient!!
49 percent wanted to extract this molar with the wisdom tooth! In a short 6 months we see incredible healing! Look at the bone fill! Time to think about saving these teeth when in question!
25 percent asked wanted to extract this tooth! This is a 3 month healing film. Please explain why root amputation of a fractured mb root is not a first option when there are 2 healthy roots. I am very curious of why. This dentist(patient) flew to nyc to save this tooth because the endodontist, oral surgeon and periodontist where he lived all said to extract.
I cannot believe 49 pecent want to extract this tooth. Luckily this patient is my assistant and she knows the success of saving teeth doing root canal retreatment. This is a 1 year follow up. Healing very nicely. Please discuss why we would take this tooth out. #

Root canal on 19 was done many years ago overseas. Middle mesial found and case was underfilled. Easy to criticize. I am saying success! 18 was just completed. Lets see if we get success with an esthetic root canal!
Dont give up!! Save teeth when possible!!
