Patient came in because he thought his shoe had been rubbing on his foot from running. Biopsy showed an ulcerated Kaposi’s Sarcoma, Full body scan to be performed with an Oncology consult. 🦶

Atherosclerosis or hardening of the arteries is a calcification in the artery. When a doctor performs a physical on the foot we check for a pulse. If the pulse is not present it tells us that either the circulation is being blocked, peripheral vascular disease, or the artery is calcified and cannot pulsate anymore. Further evaluation is usually needed with either a Doppler or ankle brachial testing, possibly an angiogram of the lower extremity. In this patient you can see the calcification in the artery between the 1st and 2nd metatarsals. This patient has an ulcer and unfortunately due to his circulation is not a candidate for surgery. Stress fractures are a risk in children from their high activity level. When playing sports proper shoe wear for your foot is the most important equipment to have especially if there is a lot of running involved. The fracture can be seen in this first picture but the interesting thing is that the 2nd picture is from a week prior. This is why serial X-rays are important. These fractures don’t always show up right away and may only show up when the fracture is healing because they are so small. Why not get an mri?
