Fat Transfer: Part 3 - Making Nanofat After the fat is spun in the centrifuge, the liquid at the bottom and oil at the top are removed, leaving the pure healthy fat cells for us to process into nanofat. The fat is emulsified by passing it repeatedly between two syringes using a small coupler. Once this is complete, you are left with a homogenous gel-like substance called nanofat that is now ready to be transferred! Fat Transfer: Part 2 - Processing The Fat After harvesting the fat, it is important to isolate the healthy fat cells from any oil and extra liquid in order to maximize fat survival after transfer. There are multiple ways to separate the fat- filtration, gravity, and centrifugation are the most commonly used methods.  Spinning, the fat down in a centrifuge helps me quickly isolate the healthiest portion of fat from everything I have harvested. Fat Transfer: Part 1 - Fat Harvest The first step for fat transfer is actually harvesting the fat. Fat can be taken from many parts of the body- I’m harvesting it from the lower abdomen in this patient. I am using a hand-held manual technique which produces a low suction pressure to harvest the fat. I’ve been seeing a lot of younger patients in their mid 30’s and 40’s who are unhappy with The appearance of their neck and jawline. Let’s talk facial aging! What really happens to our faces as we age? Skin aside, just looking at deeper structures, we have attenuation and stretching of supporting ligamentous and septal structures which allows for downward migration and bulging of fat in certain areas. Some fat loss, most notably in our temples and cheeks. I’ve received a lot of questions from patients about a recent post of my patient before and after facial resurfacing. She had a TCA deep chemical peel combined with CO2 fractional laser treatment of her face, eyelids, neck, and chest. These treatments cause sloughing of the outer layer of damaged skin cells and stimulate new collagen production. Each of these alone are great at smoothing fine lines and erasing dark pigment, and when combined they are incredibly powerful at undoing skin damage and turning back the clock on skin aging. A few weeks of skin prep are required prior to the either procedure to optimize results. And you can expect about 7-10 days of peeling, swelling, and some redness but minimal to no discomfort.
