To be anti aging in the truest sense of the term, means to apply skin nutrients consistently that are ANTI inflammatory. So when selecting a cell source of growth factors and cytokines, the secretome needs to be taken into consideration. The secretome is the portfolio of proteins that a cell naturally gives off. What are some cell options?

Unlike other cells which stay in their niche tissue of origin, the bone marrow stem cell is the only healing, MOBILE, cell in the body that has the ability to travel 🚒 to the site of injury and orchestrates healing in that area. For example, when skin has an injury like a cut 🔪, the BMSC travels to the area of connective tissue and will actually tell the fibroblasts what to do. Think of the BMSC as the general and fibroblasts as the soldiers taking orders. Now, fibroblasts are in fact the most dominant cell in connective tissue responsible for extracellular matrix protein production (collagen, elastin & glycosaminoglycans). However, it's the BMSC's "message" 📲 that determines how those fibroblasts behave.

What REALLY happens to our skin in the presence of chronic inflammation?? **PART TWO....**

IMMUNOSENESENCE- if you've ever heard of the term microbiome when discussing gut health, then you may be surprised to know that we also have an entire cutaneous ecosystem as well! Microbiome is the balance of the good (commensal) microorganisms & bad (pathogenic or disease causing) microorganisms. 🦠 An entire flora family of viruses, fungi, bacteria and yeast are present to protect and defend our skin from UV radiation, environmental stressors, etc... think of an army of soldiers, each performing their specific duty. 🤺 With chronic inflammation we can lose these soldiers and our skin essentially becomes sick and begins to deteriorate. 🤒🤢

TELOMERE ATTRITION- Think deep, deep down into molecular and cellular biology to the chromosomal level. Chromosomes are where DNA strands form our genetic machinery. At each end of our chromosomes are telomeres. Imagine shoelaces and the ends with the shoelace "caps". 👟These caps are our telomeres & as we age they become shorter and shorter. Every time the cell devides, those caps shorten, thin & eventually fray. Although this inherently happens with the aging process, chronic inflammation damages our telomeres with a starting sprint resulting in cellular senesence. 🏃‍♀️

MATRIX METALLOPROTEINASES- (MMP's!) MMP's are enzymes that degrade other proteins! When the skin is wounded these enzymes play a role in tissue remodeling & other various physiological processes like angiogenesis and morphogenesis. Collagenase, for example, degrades collagen. Hyaluronidase destroys hyaluronic acid. Although MMPs play a vital role during wound healing, they should not be activated on an every day basis. This is exactly what happens with chronic inflammation- a rapid increase in MMP's which directly ages the face with loss of key structural proteins like Hyaluronic acid, collagen, elastin., etc... 🧟‍♂️🧟‍♀️

It's been established that when we consistently apply products with inflammatory properties, morning & night, over an extended period of time, chronic inflammation sets in. 🔥 But what does that REALLY mean for our skin? 🧐

Metabolic wear and tear at the molecular and cellular level!!! So what happens??

-DEJ FLATTENING- The dermal epidermal junction adheres our outermost layer, the epidermis, to our dermis. This area provides structural integrity to the epidermis and histologically, in younger skin, appears mountainous. These hills and valleys are called rete pegs. In states of chronic inflammation, rete pegs begin to flatten. This reduces surface area & inhibits transfer of oxygen, nutrients & removal of waste. All of which are crucial for skin health.

-MITOCHONDRIAL DAMAGE- "The powerhouse of the cell", if you remember biology class.💪 This structure of our cells is responsible for cellular energy called ATP (Adenosine Triphosphate). Without mitochondria and ATP we would essentially lose all function within our cells. With chronic inflammation mitochondria are a direct target. 🎯

-REACTIVE OXYGEN SPECIES- most industry providers have heard of free radical damage! As chronic inflammation sets in, free radicals run rampid. Free radicals are rogue molecules that have lost an electron. They will then proceed to steal electrons from other cells or molecules to "stabilize themselves". Remember, electrons like to remain in pairs! (& this is why we use antioxidants!) This can lead to DNA damage and difficulties in cell replication. 🙀

We know that we can truly optimize our skins functionality utilizing topically applied growth factors and cytokines. We also know the market is saturated with "the latest and greatest" technologies. So... how do we choose? Why is one growth factor product potentially better than another? Before we get to that, let's talk inflammation! ♨️

To understand anti-aging, in the truest sense of the term, would be to recognize and understand the role of inflammation. Today I want to focus on CHRONIC inflammation. Chronic inflammation occurs when inflammatory pathways are expressed consistently over a long period of time. (Immediately after a skin procedure like microneedling, for example, would be considered ACUTE inflammation.)

Chronic inflammation, a low-level simmer of inflammatory gene expression, can be rather confusing because one may not notice the typical indicators of inflammation in their skin like redness, heat, irritation etc...The surface of the skin may appear "normal" and perhaps even "improved" after beginning a regimen. Let's dive deeper. 👨‍🏫

Subclinical inflammation is underneath what we can see at the surface. We know after a period of time, a domino effect of deleterious happenings can occur. (more on this later) Chronic inflammation can certainly lead to increased edema, or swelling, which can seemingly make lines and wrinkles disappear. So ask yourself & proceed with caution when your new favorite product claims "reduce lines and wrinkles in as little as 2 weeks". Are we really stimulating collagen ,elastin, etc... that quickly? Or are we inflaming the skin underneath to give ourselves a plumped appearance? 🔥

Products that we apply consistently every morning and evening should ALWAYS be anti-inflammatory. If our topicals upregulate inflammatory pathways routinely AM/PM for a long period of time what happens?! Chronic Inflammation!!!!! Keep an eye out for the details of what ACTUALLY happens in the presence of chronic inflammation. (Hint- it's not pretty. And quite fascinating) 🤓

Growth Factors & cytokines! What are they exactly? 🤓

All cells within the human body (except 2) communicate with one another with a profound and complex language. Think of this communication as an entire symphony of cell signaling regulating biological processes.

These cell signals are the cytokines and growth factors! Kind of like a text message! Cytokines and growth factors are protein molecules that leave one cell and bind to specific receptors on the surface of their target cells and induce signaling pathways. Think of lock and key. The message received from one cell determines how its "receiving" cell will behave. (The cell source determines what signals are released.) When cell receptors are engaged, the cell then will produce more of that molecule & perhaps activate/regulate gene expression within that cell.

Growth factors and cytokines control processes required for development including tissue organization, morphogenesis, angiogenesis and are also involved in regulating metabolism, wound healing, and maintaining tissue homeostasis.

So the question is... are all Growth Factors & cytokines created equal? Are they all great for DAILY topical application? Can they pe*****te skin? How about isolated growth factors? Pore strips. Are they worth it?

1. Adhesive strips are often too gentle to remove anything but the most superficial buildup (sweat, oil, dead skin & debris). 😇 Conversely, at times, strips are so adhering that they could further compromise barrier and potentially damage the capillary network. We've all seen the tear jerking peel off masks. 😈 (Sensitive skin can also have undesired reactions to the adhesive properties themselves. 🙊)

2. If the goal is to "shrink pores", we may need to be more realistic. 😬🤦‍♂️ Unfortunately, social media has plagued society into reaching for the "I have no pores & flawless complexion" stars. ⭐ As I've mentioned in previous posts, pores are meant to exist in the functionality of the integument! ✔ And they are NORMAL! Yes, you may be obsessing in attempt to elimate something that is very natural and actually serves a purpose.

Pores do not open and close with muscular attachments, rather they can appear smaller or larger depending on the cleanliness of the pore. 🧼 There are many alternative options to clean your pores if they do get congested. (Try ingredients like salicylic acid or bentonite/kaolin clay masks 👃)

*Also be aware that sebaceous filaments and blackheads are very different. Many people try to remove their filaments with pore strips when that too, can be more deleterious than helpful. ⚠️ Not to mention, 24 hours later you'll likely be right back where you started. ⏰*

3. Let's not put a temporary bandaid on an underlying problem. If you are experiencing blackheads- prevention is key! 🔑 Using a smart retinoid is one of my favorite ways to keep plugs from forming. 🧠 (Try topicals like Differin gel or Retinol. Or, in more severe cases, discuss prescription options with your Dermatologist. 👩‍⚕️) Remember, you can always do too much of a good thing. Keep your barrier in optimal function! )

Struggling with cracked corners of the mouth?? It may not be what you think! I have seen a LOT of this over the last several weeks so I thought I'd share.....
Angular cheilitis is a condition that causes red, swollen patches in the corners of your mouth where your lips meet and make an angle. Other names for it are perleche and angular stomatitis. You can get it on one side of your mouth or on both sides at the same time.
Saliva potentially can get trapped and builds up in the corners of your mouth. When it dries, the skin in the area can crack. (You may be licking your lips often to soothe your cracked skin.) The warmth and moisture in the corners of your mouth create the perfect conditions for fungus to grow and multiply -- and cause infection
Fungal infection is the most common cause of angular cheilitis. It’s usually caused by a type of yeast called Candida-- the same fungus that causes diaper rash in babies. Certain bacteria strains or a deficiency in riboflavin (vitamin B2) may also lead to angular cheilitis.  
Make sure to schedule with your dermatologist for proper diagnosis and treatment recommendations. I also have tips and tricks to help. DM me for guidance!

🐍 A couple of weeks ago I did a ! I hadn't done a peel service in almost 3 years so I decided to go ahead with a fresh start! Here you can see what my most intense day of peeling looked like.
My favorite part of this treatment is that it is comfortable, controlled and predictable. Although we can visibly see skin cell proliferation, the skin never appeared red, felt sore or tender. 1 week after the peel solution was applied my skin felt smoother, the appearance of pores reduced, tone became more even and radiance amplified. 😇
Peel services to this degree are not for everyone and I'm very particular who I feel is a candidate for a treatment like this. Barrier function is extremely important. 💪
